枕下经幕上入路显微手术治疗天幕脑膜瘤策略

Microsurgery for occipital transtentorial approach in the treatment of tentorial meningioma

摘要 目的探讨适合枕下经幕上入路的影像学分型及显微手术策略。方法回顾性分析我院神经外科2016年5月至2019年9月枕下经幕上入路切除的10例天幕脑膜瘤患者的临床资料、首发症状、影像学分型、肿瘤大小。结果按照Simpson分级标准评价肿瘤切除程度,I级切除2例,II级切除7例,IV级切除1例。术后1例出现中枢神经系统感染,1例出现肺部感染,治疗后恢复,无死亡病例,术后患者症状均较术前改善,随访期内所有患者正常生活。结论枕下经幕上入路适合于治疗镰幕区、瘤体位于幕上向镰幕区延伸的旁正中型、瘤体处于幕上的窦汇区脑膜瘤。 Objective To investigate the imaging classification and microsurgical treatment strategy suitable for occipital transtentorial approach.Methods Clinical date,first symptom,image classification,extent of the tumor resection,follow-up of 10 cases with tentorial meningioma who adopted occipital transtentorial approach during May 2016 to September 2019 in Department of Neurosurgery of our hospital were retrospectively analyzed.Results According to Simpson’s granding for tumor removal,grade I removal was performed in 2 cases,grade II removal was performed in 7 cases,grade IV removal was performed in 4 cases.One patient developed central nervous system infection and one patient developed pulmonary infection after the operation.They were recovered after treatment.There were no death cases.The postoperative symptoms of the patients were all improved compared with those before the operation.During the follow-up period,all the patients lived normal lives.Conclusion Occipital transtentorial approach is suitable for treatment of tentorial meningioma where in the falcotentorial junction,between free and lateral edges which located on the tentorial near falcotentorial junction and confluens sinus meningiomas which located on the tentorial.
