摘要
近年来导师在大量冠心病人群诊治工作中,发现痰瘀互阻型已逐渐成为冠心病患者的主流证型,尽管大部分冠心病患者的证型较为复杂,或伴气虚、阳虚,或伴气滞,或伴寒凝,但瘀血、痰湿相互搏结仍是主要的证治特点,二者互为因果,瘀滞心脉,且伴随整个发病阶段。汉·张仲景所著《金匮要略》中指出胸痹的基本病机为"阳微阴弦",诸代医家对此有不同的解读,对胸痹治疗有重要的指导意义。文章通过大量的文献研究,从胸痹的古籍论述、"阳微阴弦"理论源流、"阳微阴弦"继承及发扬、"阳微阴弦"对化痰祛瘀方指导等方面来一一论述,为治疗冠心病提供更多的思路。
The diagnosis and treatment of a large number of patients with coronary heart disease in recent years,my tutors have found that phlegm and blood stasis stagnation has gradually become the mainstream syndrome type of patients with coronary heart disease.Although most of the patients with coronary heart disease have complex syndrome types,such as Qi deficiency,Yang deficiency,Qi stagnation or cold coagulation,blood stasis and phlegm-dampness stagnation are still the main syndrome treatment characteristics.Point,they are cause and effect each other,stasis of heart and vein,and accompanied by the whole stage of disease.In Synopsis of the Golden Chamber written by ZHANG Zhongjing,it is pointed out that the basic pathogenesis of thoracic arthralgia is“weak pulse at Yang and stringy pulse at Yin”,which has been interpreted differently by doctors of different generations and has important guiding significance for the treatment of thoracic arthralgia.Through a large number of literature studies,this article discusses the ancient books of chest arthralgia,the origin of the theory of“weak pulse at Yang and stringy pulse at Yin”,the inheritance and development o“f weak pulse at Yang and stringy pulse at Yin”,and the guidance of“weak pulse at Yang and stringy pulse at Yin”to Huatan Quyu Decoction(化痰祛瘀方),so as to provide more ideas for the treatment of coronary heart disease.
