摘要
目的研究自拟方调经止痛丸联合腹腔镜手术治疗子宫内膜异位症(EMS)对术后复发、输卵管功能及妊娠结局的影响。方法选取102例EMS患者采用完全随机对照方法分为对照组53例(腹腔镜手术治疗),观察组49例(术后联合自拟方调经止痛丸辅助治疗),分别随访术后3个月、6个月、9个月、12个月的复发率与受孕率情况观察。结果治疗后,观察组、对照组临床有效率分别为93.88%(46/49)和79.25%(42/53),观察组高于对照组(P<0.05);两组患者的少腹疼痛、月经血块、盆腔内结节、腰膝酸软等症状积分均明显降低(P<0.05),且观察组降低幅度更大(P<0.05);治疗后随访的3、6、9、12个月内观察组妊娠率高于对照组,复发率低于对照组(P<0.05),差异均有统计学意义;两组治疗后均未发生较为严重的不良反应,且差异比较无统计学意义(P>0.05)。结论调经止痛丸联合腹腔镜手术对于EMS合并不孕患者的治疗方案疗效确切,可显著降低EMS术后的临床复发率,一定程度上提高术后的妊娠率,值得临床推广。
Objective To study the effect of self-made Tiaojing Zhitong Pills(调经止痛丸)combined with laparoscopic surgery on postoperative recurrence,fallopian tube function and pregnancy outcomes of endometriosis(EMS).Methods A total of 102 patients with EMS were selected and completely randomized into control group(53 cases,laparoscopic surgery)and observation group(49 cases,laparoscopic surgery combined with self-made Tiaojing Zhitong Pills adjuvant treatment after surgery).The recurrence rate and pregnancy rate were observed at 3,6,9 and 12 months of follow-up after surgery.Results After treatment,the clinical effective rate was93.88%of the observation group,higher than 79.25%of the control group(P<0.05).The scores of symptoms such as lower abdominal pain,menstrual blood clots,pelvic nodules and soreness and weakness of waist and knees were significantly reduced in the two groups(P<0.05),and the reductions were even greater in the observation group(P<0.05).Within 3,6,9 and 12 months of follow-up after treatment,the pregnancy rate in the observation group was higher than that in the control group while the recurrence rate was lower than that in the control group(P<0.05).No serious adverse reactions occurred in the two groups after treatment(P>0.05).Conclusion Tiaojing Zhitong Pills combined with laparoscopic surgery has an exact efficacy in the treatment of patients with EMS complicated with infertility.It can significantly reduce the clinical recurrence rate after EMS surgery and increase the pregnancy rate after surgery to a certain extent,and thus it is worthy of clinical promotion.
