摘要
目的通过对合并桥本氏甲状腺炎(HT)及甲状腺功能正常的多囊卵巢综合征(PCOS)患者的一般临床特征和实验室化验指标进行差异性分析,探究TgAb及TpoAb在多囊卵巢综合征患者中的差异,为疾病的进一步诊疗提供依据。方法回顾2015年3月—2018年8月黑龙江中医药大学附属第一医院妇科门诊临床科研信息一体化系统中,首次诊断为PCOS患者780例,根据TgAb及TpoAb值分为4组,进行一般临床特征和实验室化验指标的差异性分析。结果HT组与非HT组在舒张压、T、SHBG、TG、TC和APOB值方面差异有统计学意义(P<0.05);在进行不同表型的甲状腺抗体比较中TpoAb组在WHR、FSH、LH/FSH、DHEAS、AND、FPG、FINS和LDL方面与其他3组差异有统计学意义(P<0.05)。结论合并HT或TpoAb的PCOS患者性激素和糖脂代谢方面存在相对严重紊乱的状态,甲状腺抗体与PCOS患者的内分泌代谢之间存在一定的联系。
Objective To analyze the general clinical features and laboratory tests of patients with polycystic ovary syndrome(PCOS)with Hashimoto’s thyroiditis(HT)and normal thyroid function and explore the difference between TgAb and TpoAb in patients with PCOS and provide a basis for further diagnosis and treatment of the disease.Methods A total of 780 patients first diagnosed as PCOS from Mar.2015 to Aug.2018 in Department of Obstetrics and Gynecology of The First Affiliated Hospital of Heilongjiang University of Chinese Medicine were collected and randomly divided into four groups based on TgAb and TpoAb.Then we analyzed the general clinical characteristics and laboratory test indicators.Results Therewere significant differences in diastolic blood pressure,T,SHBG,TG,TC and APOB between HT group and non-HT group(P<0.05).There were significant differences in WHR,FSH,LH/FSH,DHEAS,AND,FPG,FINS and LDL of the TpoAb group among in the comparison of thyroid antibodies with different phenotypes(P<0.05).Conclusion There is a relatively serious disorder in the metabolism of sex hormones and glycolipids in PCOS patients with HT or TpoAb.There is a link between thyroid antibodies and endocrine metabolism in patients with PCOS.
