摘要
目的了解妊娠对心电图的影响。方法记录51例研究对象(平均29.5±4.6岁)孕前期、孕中期、孕晚期的12导联同步心电图,测量每份心电图的心率、心电轴、P波时限、PR间期、QRS时限、QT间期、Ⅱ和V_(1)导联的P波振幅、各个导联Q波、R波、S波、T波振幅,比较各项指标在孕前期、孕中期、孕晚期的变化。结果十二导联心电图的心率、心电轴、P波时限、PR间期,QRS时限,QT间期等时间相关的数据在孕前、孕中、孕晚期没有显著差异(P>0.05)。Ⅰ导联R波振幅在孕晚期大于孕前和孕中期(均P<0.01),aVL导联R波振幅孕晚期均大于孕前(P<0.01)。V_(1)导联S波振幅在孕晚期小于孕前(P<0.05),V_(2)导联S波振幅在孕中期小于孕前(P<0.05)。V_(1)导联R/S在孕晚期大于孕前期和孕中期(P<0.05),V_(2)导联R/S在孕晚期大于孕前(P<0.05)。Ⅰ、aVR、aVL导联T波振幅在孕晚期大于孕前(P<0.01)和孕中期(P<0.05),Ⅲ和V_(1)导联T波振幅孕晚期大于孕前(P<0.05)。V_(2)导联T波振幅孕中期和孕晚期小于孕前(P<0.05)。结论妊娠对心电图的影响主要表现为孕晚期Ⅰ、aVL导联R波振幅、T波振幅以及V_(1)、V_(2)~V_(6)导联R/S增大,为妊娠晚期子宫压迫心脏使其呈横位以及心脏负荷增加所致。
Objective To understand the effect of pregnancy on the ECG.Methods Twelve-lead synchronized ECG of 51 study subjects(29.5±4.6)years old in the first trimester,second trimester and third trimester were recorded.The heart rate,ECG axis,P-wave time,PR interval,QRS duration,QT interval,and QT interval were measured on each ECG.The P amplitude of leadⅡand V_(1)and Q wave,R wave,S wave,T wave amplitude on each lead were measured.Analyze the changes of these indicators in the first trimester,second trimester and third trimester.Results On 12-lead ECG,heart rate,ECG axis,P-wave time,PR interval,QRS duration,QT interval,etc.Data did not differ significantly in the first trimester,second trimester,and third trimester(P>0.05).R wave amplitude on leadⅠin the third trimester was greater than that in the first and second trimesters(both P<0.01),and R wave amplitude on lead aVL in the third trimester was greater than that in the first trimester(P<0.01).The S wave amplitude on lead V_(1)in the third trimester was smaller than that in the first trimester(P<0.05),and the S wave amplitude on lead V_(2)in the second trimester is smaller than that before pregnancy(P<0.05).The RS ratio on lead V_(1)in the third trimester was greater than that in the first and second trimesters(P<0.05),and the RS ratio on lead V_(2)in the third trimester was greater than that in the first trimester(P<0.05).On leadⅠ,aVR,aVL,T wave amplitude in the third trimester of pregnancy was greater than that in the first trimester(P<0.01)and the second trimester(P<0.05).The T amplitudes of leadⅢand V_(1)in the third trimester pregnancy was greater than that before pregnancy(P<0.05).T wave amplitude on lead V_(2)in the second and third trimesters of pregnancy is smaller than that before pregnancy(P<0.05).Conclusions The effect of pregnancy on ECG is mainly manifested as the increase in the R wave and T wave amplitude on leadⅠand aVL,and the RS ratio on the lead V_(1)and V_(2)in the third trimester.It is caused by the uterine compression of the heart in the later trimester of pregnancy to make it transverse,and the increase in cardiac load.
