摘要
目的探讨高分辨率磁共振血管壁成像(HR-VWI)在评价中枢神经系统血管炎(CNSV)活动性的应用价值。方法21例临床确诊为CNSV的患者分别在活动期和非活动期行HR-VWI增强检查。以节段为单位统计患者血管受累情况。比较活动期和非活动期血管壁的表现特点,包括"环征"、血管壁最大厚度、血管壁强化程度、管壁强化的信噪比(SNR)和对比噪声比(CNR)。组间计数资料采用McNemar检验,计量资料采用配对样本t检验。结果活动期和非活动期的血管壁呈"环征"的节段分别为40/91、19/91,血管壁明显强化的节段分别为45/91、25/91,轻度强化的节段分别为38/91、55/91,差异均有统计学意义(P<0.05)。患者活动期组受累节段血管壁最大厚度(1.72±0.43)mm,明显大于非活动期组(1.46±0.50)mm,活动期和非活动期管壁强化的SNR值分别为(242.9±73.2)和(207.0±68.2),CNR值分别为(232.4±71.4)和(201.1±67.7),均存在显著差异(P<0.05)。结论HR-VWI能够观察CNSV患者活动期和非活动期血管壁表现的变化,对评估CNSV的活动性具有重要的应用价值。
Objective To explore the clinical value of high-resolution magnetic resonance vessel wall imaging(HR-VWI)in evaluating the activity of central nervous system vasculitis(CNSV).Methods The plain scan and enhanced scan of 3 D CUBE T1 technique were performed in 21 patients with clinically confirmed central nervous system vasculitis in the active and inactive phases,respectively.The vascular involvement of the patients was counted by segments.The features of vessel wall in active and inactive phases were compared,including ring sign,the maximum thickness of the vessel wall,enhancement degree of the vessel wall,signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR).The Mc Nemar test was used for enumeration data between groups.A paired-sample t test was applied for measurement data.Results The segments with ring sign in active and inactive phases were 40/91 and 19/91,respectively.The segments with obvious enhancement were 45/91 and 25/91,while the segments with slight enhancement were 38/91 and 55/91 in the active and inactive phases.Therefore,the difference was statistically significant(P<0.05).The maximum thickness of the vessel wall in the active group was(1.72±0.43)mm,which was significantly greater than that in the inactive group(1.46±0.50)mm.The SNR value of the enhancement of the vessel wall in the active group and the inactive group were(242.9±73.2)and(207.0±68.2),and CNR value were(232.4±71.4)and(201.1±67.7)respectively,with significant differences(P<0.05).Conclusion High-resolution magnetic resonance vascular wall imaging can observe the changes of vascular wall in the active and inactive phases,hence providing valuable information to evaluate the activity of CNSV.
