摘要
目的探讨髋关节软骨MR定量体积测量的可靠性和不同程度软骨损伤的髋关节骨关节炎(OA)患者软骨体积变化特征的定量评估可行性。方法纳入138例临床诊断为OA的患者行三维(3D)-双回波稳态(DESS)MRI扫描,根据其软骨损伤情况将117例纳入患者分为正常软骨对照组(n=37)、轻度软骨损伤组(n=35)、重度软骨损伤组(n=45)。对股骨、髋臼软骨进行自动软骨分割和体积测量,比较三组间股骨、髋臼各亚区软骨体积及体积百分比的差异。结果股骨前上、股骨中前上、上后侧区域及髋臼前上区和中间区,重度软骨损伤者体积减少,具有显著统计学差异,P<0.05。结论基于计算机软件的自动软骨分割技术可实现对髋关节软骨体积的定量评估,本研究发现重度软骨损伤患者软骨体积较正常和轻度软骨损伤患者明显降低,在OA的疾病病程中伴随着关节软骨体积的减少。对OA患者软骨体积改变进行纵向定量评估,可实现对OA疾病病程的动态监测。
Objective To investigate the reliability of quantitative MR volumetric measurement of hip cartilage and to evaluate the feasibility of quantitative assessment of cartilage volume changes in OA patients with different degrees of cartilage damage.Methods 138 patients with OA were examined with 3 D-DESS-MRI,according to the cartilage injury,117 patients were divided into normal cartilage control group(n=37),mild cartilage injury group(n=35),severe cartilage injury group(n=45).Automatic segmentation of hip femur and acetabular cartilage was done,then the difference of cartilage volume and volume percentage among the three groups was analyzed.Results The volume of the patients with severe cartilage injury was reduced.There were statistically significant differences in the areas of anterior upper femur,anterior upper central upper and posterior upper femur,anterior upper acetabulum and middle acetabulum(P<0.05).Conclusion The automatic cartilage segmentation technology based on computer software can achieve quantitative evaluation of the cartilage volume of the hip.Our study found that the volume of patients with severe cartilage injury was significantly lower than that of normal and mild cartilage injury patients,and the volume of articular cartilage was reduced in the course of OA.The longitudinal quantitative evaluation of the change of cartilage volume in OA patients can be useful for dynamic monitoring of the course of OA disease.
