动态MRI与动力位X线片在测量颈椎椎体间角度的对比分析 被引量:2

Comparative Analysis of Dynamic MRI and Dynamic Position X-ray Film in Measuring the Angle between Cervical Vertebrae

摘要 目的分析颈椎动态MRI与颈椎动力位X线片在测量颈椎椎体间角度中的差异性及相关性,以期了解动态MRI是否可用于评估颈椎的稳定性。方法回顾性分析2016年1月至2018年6月诊治的3天内同时接受颈椎动态MRI与颈椎动力位X线检查且资料完整、影像学资料清晰的患者44例,男24例,女20例,年龄32~63岁,平均(46.27±7.34)岁。在两种检查方法上测量颈椎各节段的椎体间角度,采用配对样本t检验比较两种检查方法的差异性,采用Pearson相关分析对在两种检查方法上测量的椎体间角度进行相关性分析,同时采用线性回归建立各椎体间角度在不同检查方法的线性模型。结果在两种检查方法上测量的颈椎各节段椎体间角度的差异均无统计学意义(P>0.05)。在颈椎MRI与颈椎X线片三个位置(中立位、过屈位、过伸位)上测量的各节段颈椎椎体间角度之间在统计学上存在强到极强的正相关(P<0.05)。线性回归分析还显示动态MRI与动力位X线片上测量的各节段椎体间角度之间在统计学有显著的相关性(P<0.05,R^20.375~0.923)。结论颈椎动态MRI上测量的颈椎各节段的椎体间角度与动力位X线片上测量的结果相近,颈椎各节段椎体间角度在动态MRI及动力位X线片上存在正相关,因此在一定程度上颈椎动态MRI可代替颈椎动力位X线片测量椎体间角度,并可用于评估颈椎的稳定性,此外亦可当做判断孕妇等人群颈椎稳定性的替代方案。 Objective To analyze the difference and correlation between cervical dynamic MRI and cervical dynamic radiographs in measuring the angle between cervical vertebrae.Methods A retrospective analysis of 44 patients who underwent cervical dynamic MRI and cervical dynamic X-ray examination and complete data and clear imaging data within 3 days from January 2016 to June 2018.The subjects included 24 males and 20 females,age 32-63(46.27±7.34)years old.The angle between the vertebral bodies of each segment of the cervical vertebrae on the two methods of examination were measured.The paired sample t test was used to compare the differences between the two methods.Pearson correlation analysis was used to analyze the correlation between the interbody angles measured on the two examination methods.At the same time,linear regression was used to establish a linear model of the angle between the various vertebral bodies in different inspection methods.Results There was no significant difference in the angle between the vertebral bodies of the cervical vertebrae measured by the two methods(P>0.05).There was a statistically strong positive correlation between the cervical vertebrae angles measured at three locations of cervical vertebra MRI and cervical X-ray(P<0.05).Linear regression analysis also showed a statistically significant correlation between dynamic MRI and the intervertebral angles measured on the dynamic radiographs(P<0.05,R2=0.375-0.923).Conclusion The intervertebral angles of the cervical vertebrae measured by dynamic MRI on the cervical vertebrae were similar to those measured on the dynamic X-ray film.There is a positive correlation between the intervertebral angles of the cervical vertebrae on the dynamic MRI and the dynamic radiograph.Therefore,to a certain extent,dynamic cervical MRI can replace the cervical vertebra dynamic position X-ray film to measure the interbody angle,and can be used as an alternative to determine the stability of cervical vertebrae in pregnant women and other groups.
