摘要
目的探讨CT不同征象在结节性甲状腺肿与甲状腺腺瘤中鉴别诊断的价值。方法回顾性分析经手术及病理证实74例甲状腺良性结节CT影像表现,包括结节外甲状腺腺体改变(体积、密度、轮廓)、结节数目、结节大小、结节密度、结节边界、结节部位、结节强化程度、结节强化方式、结节内钙化、周围气管、淋巴结及远处转移。结果本组74例,单发结节26例,多发结节48例;其中结节性甲状腺肿62例,甲状腺腺瘤12例。结节性甲状腺肿、甲状腺腺瘤在结节数目、结节部位、结节边界、结节强化程度及结节外甲状腺腺体体积、轮廓等差异有统计学意义(P<0.05),在结节大小、结节密度、结节钙化、结节强化方式、增强后结节边界较平扫清晰及结节外甲状腺腺体密度方面无统计学差异。结论掌握结节性甲状腺肿与甲状腺腺瘤CT的各种征象及结节外甲状腺腺体结构改变,对诊断甲状腺良性结节具有一定的价值。
Objective Explore the value of different signs of CT in differential diagnosis of nodular goiter and thyroid adenoma.Methods Imaging findings of 74 benign thyroid nodules confirmed by surgery and pathology were retrospectively analyzed,including changes in the thyroid gland(volume,density,contour),number of nodules,nodule size,nodule density,nodule boundary,nodule site,nodule enhancement amplitude,nodule enhancement mode,intra-nodular calcification,surrounding trachea,lymph node,and distant metastasis.Results There were 74 cases,26 single nodules and 48 multiple nodules,including 62 nodular goiter and 12 thyroid adenoma.The differences between Nodular goiter and thyroid adenoma in the number of nodules,nodule site,nodule boundary,nodule enhancement amplitude and nodule outer thyroid gland volume,contour and other differences were statistically significant(p<0.05).There was no statistical difference in nodule size,nodule density,nodule calcification,nodule enhancement mode,nodule boundary after enhancement and extra-nodular thyroid gland density.Conclusion Understanding the various CT signs of nodular goiter and thyroid adenoma and the structural changes of the extra-nodular thyroid gland has certain value in the diagnosis of benign thyroid nodules.
