摘要
目的探讨不同对比剂注射参数和螺距在双上肢动脉CT血管造影(CTA)成像中去除静脉对比剂伪影的价值。方法连续选取临床行双上肢动脉CTA的患者80例,按照不同的对比剂、盐水注射流率和螺距随机分为四组,每组20例。A组,对比剂和盐水注射流率均为5.0 ml/s,螺距1.2;B1组,对比剂和盐水注射流率均为3.5 ml/s,螺距1.2;B2组,对比剂注射流率3.5 ml/s,追加盐水流率5.0 ml/s,螺距1.2;B3组,注射对比剂流率3.5 ml/s,盐水流率5.0 ml/s,螺距0.8。四组其余扫描参数和重组参数均相同。分别测量四组重组图像的上肢动脉内强化CT值、主动脉的强化CT值及头臂静脉CT值,比较组间的差异性。图像质量的主观评分采用4分法评价,进行非参数检验。结果68例患者符合纳入标准进入本研究。四组患者的年龄、体重指数(BMI)差异无统计学意义。四组图像质量评分差异有统计学意义,其中B3组图像质量评分最佳。四组肱动脉、腋动脉、锁骨下动脉、升主动脉的强化CT值比较没有差异性。结论在进行双上肢动脉CTA成像时,按照螺距0.8、对比剂流率3.5 ml/s,盐水流率5.0 ml/s的扫描方案可以降低静脉硬化伪影的干扰,得到清晰的上肢动脉图像。
Objective To investigate the value of different pitch and contrast injection rate in removing CTA contrast artifact in double upper limb images.Methods A total of 80 patients with double upper limb arterial CTA were selected.They were randomly divided into four groups according to different contrast agents,saline injection rate and pitch,with 20 patients in each group.In A group,the contrast agent and the saline injection rate was 5.0 ml/s,pitch 1.2;group B1,contrast agent and saline injection rate was 3.5 ml/s,pitch 1.2;group B2,injection rate of contrast medium 3.5 ml/s,saline flow rate 5.0 ml/s,pitch 1.2;group B3,contrast agent injection rate of 3.5 ml/s,water flow rate 5.0 ml/s,pitch 0.8.The four groups had the same scanning parameters and reconstruction parameters.The enhanced CT value of the upper limb artery,the enhanced CT value of the aorta and the CT value of the brachiocephalic vein were measured in 4 groups of reconstructed images,and the difference between the two groups was compared.The subjective score of image quality was evaluated by 4 point method and nonparametric tests were carried out.Results 68 patients met the inclusion criteria and were enrolled in this study.There was no significant difference in the age and weight between the 4 groups.The difference of image quality score between the 4 groups was statistically significant.Among them,the B3 group had the best image quality score.There was no difference in the CT value of the brachial artery,axillary artery,subclavian artery and ascending aorta between the 4 groups.Conclusion When performing CTA imaging of the upper limb arteries,according to the 0.8 pitch,the contrast agent flow rate of 3.5 ml/s and saline flow rate of 5.0 ml/s can reduce the interference of venous sclerosis artifacts and obtain a clear upper limb artery image.
