妊娠期高血压对早产儿脑发育影响的DTI评估 被引量:4

DTI Assessment of the Effects of Gestational Hypertension on Brain Development in Preterm Infants

摘要 目的采用扩散张量成像(DTI)评估母亲妊娠期高血压对早产儿脑发育影响。方法选取进行DTI筛查的早产儿78例,妊娠期高血压疾病孕妇分娩的36例早产儿作为观察组,另42例无高危围产因素孕妇分娩的早产儿作为对照组,应用3.0 T磁共振扫描仪对所有研究对象行常规MRI序列扫描和DTI扫描,分析两组围生期资料、颅脑损伤情况和脑白质微结构的改变。对病例组额叶白质FA值与出生时Apgar评分进行相关性分析,计算相关度。结果两组早产儿的内囊后肢、胼胝体压部、额叶白质、顶叶白质、大脑脚FA值平均值差异有统计学意义(均P<0.05),病例组早产儿额叶白质FA值与出生后1 min、5 min Apgar评分无明显相关性(r=0.244,P>0.05;r=0.273,P>0.05)。结论母亲妊娠期高血压疾病可导致早产儿脑发育落后,以磁共振DTI序列检测内囊后肢、胼胝体压部、额叶白质、顶叶白质以及大脑脚的FA值对于评估母亲妊娠期高血压疾病对早产儿脑发育的影响具有重要的价值。 Objective Diffusion tensor imaging(DTI)is used to evaluate the effect of maternal hypertensive hypertension on brain development in premature infants.Methods From February 2018 to April 2019,78 preterm infants who underwent DTI screening in the neonatal,and 36 premature infants who were born to pregnant women with hypertensive disorder were observed.In the group,42 preterm infants who were born in high-risk perinatal factors were used as control group.All subjects underwent routine MRI sequence scan and DTI sequence scan with 3.0T magnetic resonance scanner.The perinatal data and brain were analyzed for damage and changes in brain white matter microstructure.Correlation analysis was performed on the white matter FA value of the frontal lobe and the Apgar score at birth,and the correlation was calculated.Results There were significant differences in the mean values of FA values between the posterior limbs of the premature infants,the corpus callosum,the white matter of the frontal lobe,the white matter of the parietal lobe,and the cerebral peduncle(P<0.05).There was no significant correlation between the FA value of the white matter in the frontal lobe of the premature infants and the Apgar score at 1 min and 5 min after birth(r=0.244,P>0.05;r=0.273,P>0.05).Conclusion Maternal hypertensive disorder in pregnancy can lead to regressed brain development in premature infants.FA values of internal capsule posterior limb,corpus callosum,frontal white matter,parietal white matter,and cerebral peduncle are measured by magnetic resonance DTI sequence.It has important value for the influence of brain development in premature infants.
