国产VENUS-A主动脉瓣膜支架治疗老年重度主动脉瓣狭窄初步探讨 被引量:2

Preliminary Discussion of Transcatheter Home-Made VENUS-A Aortic Valve Replacement in Elder Patients with Sever Aortic Valve Stenosis

摘要 目的探讨经导管置换国产VENUS-A主动脉瓣膜支架治疗老年钙化性重度主动脉瓣狭窄(AS)的效果,为国产VENUS-A主动脉瓣膜支架在国内开展积累经验,并进一步探索经导管主动脉瓣置换(TAVR)临床应用价值。方法搜集2016年3月以来本院年龄70周岁以上严重钙化性重度AS的患者8例,术前经多学科严格评估,不能耐受外科手术,选择经导管行国产VENUS-A主动脉瓣膜置换。结果8例患者均成功完成主动脉瓣膜置换手术,术后测量主动脉瓣开放面积增加,人工瓣膜启闭功能正常,平均跨瓣压力阶差显著下降,左心室收缩功能改善。经6~12个月随访,患者临床相关症状缓解,未出现严重并发症。结论经导管置换国产VENUS-A主动脉瓣膜可用于老年性钙化性AS患者,能够改善相关临床症状,提高患者生活质量,具有很好的临床应用前景。 Objective To explore the effect of transcatheter replacement of domestic VENUS-A aortic valve stent on elderly patients with calcified severe aortic valve stenosis,to gain experience for domestic VENUS-A aortic valve stent in China,and to further explore the clinical application value of TAVR.Methods Eight patients with severe calcified aortic valve severe stenosis who were admitted to our hospital since March 2016 and were over 70 years old were selected.Before surgery,they were rigorously evaluated by multiple subjects and conclude that they may not tolerate surgical operations for Aortic valve replacement.Results All 8 patients successfully completed aortic valve replacement surgery.After operation,the aortic valve opening area was increased,the prosthetic valve opening and closing function was normal,the average transvalvular pressure difference was significantly reduced,and the left ventricular systolic function was improved.After 6-12 months of follow-up,the patient’s clinically-related symptoms improved without serious complications.Conclusion Transcatheter replacement of domestic VENUS-A aortic valve can be used in elderly Chinese patients with calcified aortic valve stenosis.It can improve related clinical symptoms and improve the quality of life of patients.It has good clinical application prospects.
