摘要
目的探讨磁共振小视野(FOV)表面线圈和BLADE序列在踝关节距骨上关节面软骨的应用价值。方法20名健康志愿者和43例患者在Siemens Avanto 1.5 T磁共振上接受MR序列检查:序列1:靴形线圈常规序列的大FOV的快速自旋回波质子密度加权成像(FSE-PDWI);序列2:靴形线圈小FOV的FSE-PDWI;序列3:靴形线圈小FOV的FSE-PDWI-BLADE序列;序列4:小线圈小FOV的FSE-PDWI;序列5:小线圈小FOV的FSE-PDWI-BLADE序列。在20名志愿者踝关节MR检查后客观比较5种MR影像的距骨软骨的信噪比(SNR)、对比噪声比(CNR);主观比较志愿者组和患者组在5种序列上的软骨大体成像是否存在差异;43例患者踝关节软骨损伤5种序列影像在软骨损伤位置、软骨损伤程度、软骨下骨、骨水肿形态、伪影方面的显示进行评分。结果在客观比较中序列1有最高的SNR与CNR,而序列5在所有小FOV序列里SNR与CNR数值最高,序列2的软骨SNR与CNR测量值最低,两两比较都有统计学差异(P﹤0.05);显示效果在志愿者组除序列2以外的其他序列评分皆高于患者组,但在全部的5种序列上两组均未表现出统计学上的差异(P﹥0.05);两名医师对踝软骨损伤显示的主观评分Kappa=0.87。使用小线圈的序列4和5主观评分高于对应靴形线圈相同序列2和3,使用BLADE序列3和5主观评分高于对应的未使用BLADE相同序列2和4,序列5在影像显示质量的5个方面评分都最高,仅在伪影评分上与序列3未表现出统计学差异(P=0.838),对其他序列均表现出统计学差异(P﹤0.05)。结论在小FOV表面线圈上选择小FOV成像参数联合应用BLADE序列能更好地显示距骨软骨。
Objective To evaluate the application value of magnetic resonance small field of view(FOV)surface coil and BLADE sequence of the talar dome articular cartilage of the ankle joint.Methods 20 healthy volunteers and 43 patients underwent the following MR sequence examination on Siemens Avanto 1.5 T magnetic resonance:Sequence 1:large sequence FOV of the conventional sequence of boot coil(BC)Imaging(fast-spin-echo-proto-density FSE-PD)sequence 2:the small FOV and FSE-PD sequence method using boot coil;sequence 3:the small FOV and FSE-PD-BLADE sequence method using boot coil;sequence 4:the small FOV and FSE-PD sequence method using small FOV surface coil;sequence 5:the small FOV and FSE-PD-BLADE sequence method using small FOV surface coil.Objective comparison of the signal-noise ratio(SNR)and contrast-to-noise ratio(CNR)were used on talar cartilage images of 5 MR sequences of the ankle joint in 20 volunteers.Subjective comparison:To compare the difference of the gross cartilage imaging between the volunteer group and the patient group on the images of five sequences.Subjective comparison of the image display quality included the displaying location of cartilage injury、the degree of cartilage injury、the shape of subchondral bone、the shape of bone edema and artifacts.Results In objective comparison,sequence 1 showed the maximum number of SNR and CNR,and sequence 5 had the highest SNR and CNR values only among all small FOV sequences.SNR and CNR measurements of sequence 2 were the lowest,there are statistical significant differences between the each two comparisons(P﹤0.05);In the subjective comparison,the display effect of the volunteer group scored higher than the patient group in all sequences except sequence 2,but there were no statistical difference between the two groups in all five sequences(P﹥0.05);the Kappa value of the subjective scoring for ankle cartilage injury by two doctors was 0.87.The subjective scores of sequences 4 and 5 using small coils are higher than the corresponding sequences 2 and 3 of the same shoe coil.The subjective scores of BLADE sequences 3 and 5 are higher than the corresponding sequences 2 and 4 of the same BLADE.The scores were the highest in all five aspects.Only the artifact score showed no statistical difference with sequence 3(P=0.838),and all other sequences showed statistical differences(P﹤0.05).Conclusion The selection of small FOV imaging parameters on the small field surface coil and the BLADE sequence can better display the talus cartilage.
