等速闭链训练与等速开链训练对脑卒中后运动障碍患者下肢肌力、平衡功能与步行能力影响研究 被引量:8

摘要 目的比较等速闭链训练与等速开链训练对脑卒中后运动障碍患者下肢肌力、平衡功能与步行能力的影响。方法选取海军军医大学附属公利医院自2019年5月至2021年5月收治的86例脑卒中后运动障碍患者为研究对象。采用随机数字表法将其分为A组与B组,每组各43例。A组采取等速闭链训练,B组采取等速开链训练。分别于治疗前与治疗后8周,记录并比较两组患者的下肢肌力[峰力矩(PT)、相对峰力矩(PT/BW)]、平衡功能(总体、前后及左右的稳定指数)与步行能力[30 s座椅站立测试(30CST)、起立行走计时测试(TUG)]。结果治疗后,两组伸肌与屈肌的PT、PT/BW均高于治疗前,差异均有统计学意义(P<0.05)。治疗后,两组患者总体、前后及左右的稳定指数较治疗前均降低,且A组低于B组,差异均有统计学意义(P<0.05)。治疗后,两组患者30CST较治疗前升高,TUG较治疗前降低,且A组30CST高于B组、TUG低于B组,差异均有统计学意义(P<0.05)。结论等速闭链训练可显著增强脑卒中运动障碍患者的下肢肌力与平衡功能,改善步行能力,效果优于等速开链训练。 Objective To compare the effects of isokinetic chain closing training and isokinetic chain opening training on lower limb muscle strength,balance function and walking ability of patients with poststroke dyskinesia.Methods A total of 86 patients with poststroke dyskinesia admitted to Gongli Hospital Affiliated to Naval Military Medical University from May 2019 to May 2021 were selected as the research subjects.They were divided into group A and group B by random number table method,43 cases in each group.Group A received isokinetic chain closing training and group B received isokinetic chain opening training.Before and 8 weeks after treatment,peak torque(PT),peak torque/body weight(PT/BW),overall,front and rear and left and right stability index,30 s chair stand test(30 CST),standing and timed up and go(TUG)were recorded and compared between 2 groups.Results After treatment,PT and PT/BW of extensor and flexor muscles in 2 groups were higher than before,the differences were statistically significant(P<0.05).After treatment,the overall,before and after and left and right stability index of 2 groups were lower than before treatment,and group A was lower than group B,the differences were statistically significant(P<0.05).After treatment,30 CST increased and TUG decreased in 2 groups compared with before treatment,and 30 CST in group A was higher than that in group B,while TUG was lower than that in group B,the differences were statistically significant(P<0.05).Conclusion Isokinetic chain closing training can significantly enhance the lower limb muscle strength and balance function of stroke patients with dyskinesia,improve walking ability,the effect is better than isokinetic chain opening training.
