摘要 目的探讨孕前体质量指数(BMI)对孕早期孕妇发生甲状腺功能减退(甲减)的影响。方法选取北部战区总医院自2020年5月至2021年6月收治的177例孕早期孕妇为研究对象。根据孕前BMI将其分为体质量过低组(BMI<18.5 kg/m^(2),n=19)、体质量正常组(18.5 kg/m^(2)≤BMI<24.0 kg/m^(2),n=103)、体质量超重组(24.0 kg/m^(2)≤BMI<28.0 kg/m^(2),n=38)、体质量肥胖组(BMI≥28.0 kg/m^(2),n=17)。根据孕早期是否发生甲减将其分为甲减组(n=25)与甲功正常组(n=152)。记录并比较不同BMI孕妇的临床资料。记录并比较甲减组与甲功正常组孕妇的临床资料。采用多因素Logistic回归分析明确孕早期孕妇发生甲减的影响因素。结果体质量正常组孕妇的促甲状腺激素(TSH)水平、甲减发生率均低于体质量过低组、体质量超重组、体质量肥胖组,差异均有统计学意义(P<0.05)。甲减组孕妇的孕前BMI、TSH水平均高于甲功正常组,游离甲状腺素(FT4)水平低于甲功正常组,差异均有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,BMI、TSH均为孕早期孕妇发生甲减的影响因素(P<0.05)。结论孕前BMI异常可增加孕早期甲减的发生,指导育龄女性最大程度的将孕前BMI控制在正常范围之内,以减少孕早期甲减的发生。 Objective To investigate the influence of prepregnancy body mass index(BMI)on hypothyroidism in pregnant women during early pregnancy.Methods A total of 177 pregnant women in the early stages of pregnancy admitted to General Hospital of Northern Theater Command from May 2020 to June 2021 were selected as the research subjects.According to their pre-pregnancy BMI,they were divided into low body weight group(BMI<18.5 kg/m^(2),n=19),normal body weight group(18.5 kg/m^(2)≤BMI<24.0 kg/m^(2),n=103),excess body weight group(24.0 kg/m^(2)≤BMI<28.0 kg/m^(2),n=38)and obese group(BMI≥28.0 kg/m^(2),n=17).According to the occurrence of hypothyroidism in early pregnancy,they were divided into hypothyroidism group(n=25)and normal thyroid function group(n=152).Clinical data of pregnant women with different BMI were recorded and compared.The clinical data of pregnant women in hypothyroidism group and normal thyroid function group were recorded and compared.Multiple Logistic regression analysis was used to determine the influencing factors of hypothyroidism in early pregnancy.Results The level of thyroid stimulating hormone(TSH)and the incidence of hypothyroidism in normal weight group were lower than those in low weight group,overweight group and obesity group,with statistical significance(P<0.05).The levels of BMI and TSH before pregnancy in hypothyroidism group were higher than those in normal thyroid function group,and the level of free thyroxine(FT4)was lower than that in normal thyroid function group,the differences were statistically significant(P<0.05).Multivariate Logistic regression analysis showed that BMI and TSH were the influencing factors of hypothyroidism in early pregnancy(P<0.05).Conclusion Abnormal BMI before pregnancy can increase the occurrence of hypothyroidism in early pregnancy,and guide women of childbearing age to control BMI before pregnancy to the maximum extent within the normal range,so as to reduce the occurrence of hypothyroidism in early pregnancy.
