摘要
目的观察疏肝健脾方联合甲基强的松龙对Graves眼病肝郁脾虚证患者眼部症状和炎症反应的影响。方法将我院110例Graves眼病肝郁脾虚证患者随机分为研究组与对照组各55例。对照组采用甲基强的松龙治疗研究组采用疏肝健脾方联合甲基强的松龙治疗。比较两组临床疗效、视力水平、突眼度、临床活动性评分(CAS)、甲状腺眼病分级标准(NOSPECS)评分、促甲状腺素受体抗体(TRAb)、抗甲状腺球蛋白抗体(TGAb)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、胰岛素样生长因子-1(IGF-1)水平。结果治疗后,研究组总有效率高于对照组(P<0.05),两组视力水平高于治疗前,突眼度、CAS评分、NOSPECS评分低于治疗前;研究组视力水平高于对照组,突眼度、CAS评分、NOSPECS评分低于对照组(P<0.05);两组TRAb、TGAb水平低于治疗前;研究组TRAb、TGAb水平低于对照组(P<0.05);两组TNF-α、IL-6、IGF-1水平低于治疗前;研究组TNF-α、IL-6、IGF-1水平低于对照组(P<0.05)。结论疏肝健脾方、甲基强的松龙对Graves眼病肝郁脾虚证患者有显著治疗作用,改善眼部症状调节免疫炎症反应。
Objective To observe the effect of Shugan Jianpi Decoction combined with methylprednisolone on ocular symptoms and inflammatory response in Graves’ophthalmopathy patients with liver depression and spleen deficiency syndrome.Methods A total of 110 patients with Graves’ophthalmopathy of liver depression and spleen deficiency syndrome were randomly divided into study group and control group,with 55 cases in each group.Control group was treated with methylprednisolone,while study group was treated with Shugan Jianpi Decoction on the basis of control group.The clinical efficacy,visual acuity,proptosis,clinical activity score(CAS),thyroid ophthalmopathy grading standard(NOSPECS),thyroxine receptor antibody(TRAb),antithyroglobulin antibody(TGAb),tumor necrosis factor-α(TNF-α),interleukin 6(IL-6),insulin-like growth factor 1(IGF-1)levels were compared between the two groups.Results After treatment,the total effective rate of study group was higher than that of control group(P<0.05);compared with before treatment,the visual acuity of both groups was higher,and was higher in study group than control group(P<0.05),while the protrusion degree,CAS score,and NOSPECS score were lower in both groups,and were lower in study group than control group(P<0.05);the levels of TRAb and TGAb in both groups were lower,and were lower in study group were than control group(P<0.05);the levels of TNF-α,IL-6 and IGF-1 in both groups were lower,and in study group were lower than control group(P<0.05).Conclusion Shugan Jianpi Decoction combined with methylprednisolone has a significant therapeutic effect on Graves’ophthalmopathy patients with liver depression and spleen deficiency syndrome,improving eye symptoms and regulating immune inflammatory response.
