摘要
目的研究血清结肠癌转移相关基因1(MACC1)、人端粒酶逆转录酶蛋白(hTERT)水平联合检测胆道良恶性肿瘤的诊断效能。方法选取2018年1月—2020年12月商丘市第一人民医院胆道恶性肿瘤患者及同期良性病变患者各90例,均检测其血清MACC1、hTERT水平,比较胆道恶性肿瘤、良性病变患者及不同病理特征血清MACC1、hTERT水平,分析血清MACC1、hTERT水平与病理特征相关性,对比血清MACC1、hTERT单独及联合检测胆道良恶性肿瘤的结果及诊断效能。结果胆道恶性肿瘤血清MACC1、hTERT水平高于胆道良性病变(P<0.05);随着胆道恶性肿瘤患者TNM分期升高、淋巴结转移,血清MACC1、hTERT水平呈逐渐升高趋势(P<0.05);经Spearman相关性分析,胆道恶性肿瘤患者血清MACC1、hTERT水平与TNM分期、淋巴结转移呈正相关(P<0.05);联合检测灵敏度(91.11%)高于MACC1检测(78.89%)、hTERT检测(77.78%)(P<0.05);MACC1检测特异度、准确度为88.89%、83.89%,hTERT检测为87.78%、82.78%,联合检测为86.67%、88.89%,三者比较差异无统计学意义(P>0.05)。结论胆道恶性肿瘤与良性病变患者血清MACC1、hTERT水平具有明显差异,且二者水平与病理特征密切相关,联合检测可提高诊断效能,有助于临床治疗方案的制定。
Objective To study the diagnostic efficacy of combined detection of serum colorectal cancer metastasis associated gene 1(MACC1)and human telomerase reverse transcriptase protein(hTERT)levels in benign and malignant biliary tumors.Methods From January 2018 to December 2020,a total of 90 patients with malignant tumor of biliary tract and 90 patients with benign lesion of biliary tract in our hospital were selected.The serum levels of MACC1 and hTERT were detected.The serum levels of MACC1 and hTERT in patients with malignant tumor of biliary tract,benign lesion of biliary tract and different pathological characteristics were compared.The correlation between serum levels of MACC1 and hTERT and pathological characteristics was analyzed.The results and diagnostic efficiency of malignant tumors.Results The levels of serum MACC1 and hTERT in patients with biliary tract malignant tumor were higher than those in patients with biliary tract benign lesions(P<0.05);With the increase of TNM stage and lymph node metastasis in patients with biliary tract malignant tumor,the levels of serum MACC1 and hTERT gradually increased(P<0.05);Spearman correlation analysis showed that the levels of serum MACC1 and hTERT in patients with biliary tract malignant tumor were positively correlated with TNM stage and lymph node metastasis(P<0.05)The sensitivity of combined detection was 91.11%higher than that of MACC1 detection 78.89%,hTERT detection 77.78%(P<0.05);The specificity and accuracy of MACC1 detection were 88.89%,83.89%,hTERT detection were 87.78%,82.78%,combined detection were 86.67%,88.89%,the difference was not statistically significant(P>0.05).Conclusion The serum levels of MACC1 and hTERT in patients with biliary malignant tumor and benign lesions are significantly different,and the two levels are closely related to the pathological characteristics.Combined detection can improve the diagnostic efficiency,which is helpful to the formulation of clinical treatment plan.
