摘要
目的探讨子宫腺肌病临床诊断中采用单纯经腹部超声检查以及联合经阴道超声检查的价值。方法选择2018年1月—2020年2月许昌市人民医院收治经病理确诊为子宫腺肌病130例患者,均采取经腹部超声检查,然后联合经阴道超声检查,比较两种方式的诊断准确率、灵敏度、特异性以及血流动力学指标。结果 (1)经腹部超声检出率为69.23%(90/130),联合检查检出率为90.77%(118/130),两种方式诊断检出率比较差异显著,有统计学意义(χ^(2)=18.8462,P=0.0000);(2)联合检查组诊断的灵敏度高于经腹部检查组,而特异性低于经腹部检查组,两种检查方式比较差异有统计学意义(χ^(2)=24.3021,P=0.0000;χ^(2)=6.1714,P=0.0130);(3)联合检查组患者的动脉搏动指数、动脉阻力指数均高于经腹部检查组,且血流量少于经腹部检查组,差异有统计学意义(P<0.05)。结论经腹部超声联合经阴道超声检查用于子宫腺肌病诊断,准确率高,为临床治疗提供可靠影像信息,值得推广。
Objective To explore the value of transabdominal ultrasonography and combined transvaginal ultrasonography in the clinical diagnosis of adenomyosis of uterus.Methods Totally 130 cases of adenomyosis were admitted to our hospital from January 2018 to February 2020,which were pathologically confirmed as uterine adenomyosis.All of them were examined by abdominal ultrasonography and then combined with transvaginal ultrasonography to compare the diagnostic accuracy,sensitivity,specificity and hemodynamic indicators of the two methods.Results(1)the detection rate of transabdominal ultrasonography was 69.23%(90/130),the detection rate of combined examination was 90.77%(118/130),and the diagnostic detection rate of the two methods was significantly different,showing statistical significance(χ^(2)=18.8462,P=0.0000).(2)the diagnostic sensitivity of the joint inspection group was higher than that of the transabdominal inspection group,while the specificity was lower than that of the transabdominal inspection group,and the comparison between the two methods was statistically significant(χ^(2)=24.3021,P=0.0000;χ^(2)=6.1714,P=0.0130);(3)the arterial pulse index and arterial resistance index of the patients in the joint inspection group were higher than those in the abdominal inspection group,and the blood flow rate was lower than that in the abdominal inspection group,with statistically significant difference(P<0.05).Conclusion Transabdominal ultrasonography combined with transvaginal ultrasonography can be used to diagnose adenomyosis of uterus with high accuracy and provide reliable image information for clinical treatment.
