摘要
目的探究老年骨质疏松与心脑血管疾病的相关性,旨在降低老年冠心病患者骨质疏松的发病率。方法回顾性分析2014年6月至2017年1月衡水市人民医院207例冠心病患者,根据双能X线骨密度结果,分为骨质疏松组(骨量减低)和非骨质疏松组(骨量正常),应用单因素相关分析比较2组患者的患者BMI指数、血压、血糖、总胆固醇、甘油三酯、高密度脂蛋白、低密度脂蛋白、血清钙、血清酶指标,并使用多因素Logisitic回归分析探究冠心病患者骨质疏松的危险因素。结果2组患者的年龄、性别、收缩压、舒张压比较无统计学差异(P>0.05);骨质疏松组BMI、空腹血糖、总肝固醇、甘油三酯、高密度脂蛋白、一氧化氮、内皮素、可溶性血管细胞黏附分子-1高于对照组,低密度脂蛋白、超氧化物歧化酶、血清钙、酸性磷酸酶、碱性磷酸酶低于非骨质疏松组,差异均具有统计学意义(P<0.05);单因素相关性分析结果显示:骨密度、血清钙、酸性磷酸酶和碱性磷酸酶均与BMI、空腹血糖、总肝固醇、甘油三酯、高密度脂蛋白、一氧化氮、内皮素、可溶性血管细胞黏附分子-1成反比,与低密度脂蛋白、超氧化物歧化酶成正比;多因素Logisitic回归分析显示:高BMI指数、高一氧化氮、内皮素、可溶性血管细胞黏附分子-1、低超氧化物歧化酶、高血糖、高血脂、低血清钙、低血清酶是冠心病患者骨质疏松的危险因素。结论老年冠心病患者骨质疏松的发病与体重指数、血糖水平、血脂水平、一氧化氮、内皮素、可溶性血管细胞黏附分子-1、超氧化物歧化酶以及血清钙、血清酶密切相关,临床上应该加以重视,及早采取干预措施降低老年冠心病患者骨质疏松的发生。
Objective To explore the correlation between osteoporosis and cardiovascular and cerebrovascular diseases in the elderly,and to reduce the incidence of cardiovascular and cerebrovascular diseases in the elderly.Methods Retrospective analysis of 207 patients with coronary heart disease in our hospital from June 2014 to January 2017.According to the results of dual-energy X-ray bone density,were divided into osteoporosis group(reduced bone mass)and non-osteoporosis group(normal bone mass).The BMI index,blood pressure,blood glucose,total cholesterol,triglycerides,high-density lipoprotein,low-density lipoprotein,serum calcium,and serum enzyme indexes were compared between the two groups.Multivariate Logisitic regression analysis was used to explore the risk factors of osteoporosis in patients with coronary heart disease.Results There was no significant difference in age,sex,systolic and diastolic blood pressure between the two groups(P>0.05);osteoporosis group BMI,fasting blood glucose,total hepatic sterol,triglyceride,high density lipoprotein,nitric oxide,Endothelin,soluble vascular cell adhesion molecule-1 is higher than the control group,low density lipoprotein,superoxide dismutase,serum calcium,acid phosphatase,alkaline phosphatase were lower than non-osteoporosis group,the differences were statistically significant(P<0.05);Single factor correlation analysis showed that bone mineral density,serum calcium,acid phosphatase and alkaline phosphatase were all associated with BMI,fasting blood glucose,total liver sterols,triglycerides,high-density lipoprotein,nitric oxide,endothelin,soluble vascular cell adhesion molecule-1 were inversely proportional to low-density lipoprotein and superoxide dismutase;Multifactorial Logisitic regression analysis showed that high BMI index,high blood sugar,high blood lipids,low serum calcium,and low serum enzymes were risk factors for osteoporosis in patients with coronary heart disease.Conclusion The incidence of osteoporosis in elderly patients with coronary heart disease is closely related to body mass index,blood sugar level,blood lipid level,serum calcium,and serum enzymes.It should be paid attention to clinically and early intervention measures should be taken to reduce the occurrence of osteoporosis in elderly patients with coronary heart disease.
