VDR表达降低对原发性胆汁性胆管炎患者免疫反应的影响

摘要 目的探讨维生素D受体(vitamin D receptor,VDR)表达降低对原发性胆汁性胆管炎(primary biliary cholangitis,PBC)患者免疫反应的影响,并分析其是否参与PBC的发生发展。方法征集2019年2月至2020年5月期间曹县县立医院住院48例确诊为PBC的患者与同期50例肝组织活检示非PBC的患者,将所有患者分别纳入观察组(均为PBC患者)与对照组(均为非PBC患者)。以实时定量聚合酶链反应(polymerase chain reaction,PCR)法检测肝脏组织和外周血淋巴细胞中VDR、miR-155 mRNA表达,蛋白质印迹法检测VDR蛋白表达,FCM型流式细胞仪检测研究对象CD4+、CD8+、CD4+8+。比较两组患者肝脏组织以及外周血淋巴细胞中上述指标的表达情况,并分析PBC患者肝脏组织VDR和miR-155、免疫指标表达的相关性。结果观察组患者VDR蛋白的表达水平与mRNA表达均低于对照组,组间比较差异显著(P<0.05);不同性别、年龄以及病程之间患者的VDR蛋白表达水平相比差异无统计学意义(P>0.05);观察组外周血淋巴细胞中VDR mRNA表达和蛋白表达均低于对照组,组间比较差异显著(P<0.05),观察组miR-155表达稍高于对照组,但差异无意义(P>0.05),经统计VDR降低组和VDR正常/升高组各有37例和11例,其中VDR降低组CD4+和CD4+/CD8+低于VDR正常/升高组,差异显著(P<0.05),而两组CD8+水平对比差异无统计学意义(P>0.05);VDR受体与miR-155呈负相关,与CD4+和CD4+/CD8+呈正相关(P<0.05),与CD8+无明显相关性(P>0.05)。结论VDR表达降低对PBC患者免疫反应产生一定影响,考虑其参与了PBC的发生发展。 Objective To investigate the effect of decreased expression of vitamin D receptor(VDR)on immune response in patients with primary biliary cholangitis(PBC),and to analyze whether VDR is involved in the occurrence and development of PBC.Methods From February 2019 to May 2020,48 patients diagnosed as PBC and 50 patients with non-PBC by liver biopsy were enrolled in the observation group(all PBC patients)and the control group(all non-PBC patients).Real-time quantitative polymerase chain reaction(PCR)was used to detect the expression of VDR and microRNA-155 in liver tissue and peripheral blood lymphocyte,Western blotting was used to detect the expression of VDR protein,FCM flow cytometry was used to detect CD4+,CD8+,CD4+8+.The expression of these indices in liver tissue and peripheral blood lymphocyte of the two groups were compared,and the correlation between the expression of VDR,micro RNA-155 and immune indices in liver tissue of PBC patients was analyzed.Results The expression level of VDR protein and mRNA in the observation group were lower than that in the control group,the difference between the two groups was significant(P<0.05).There was no significant difference in VDR protein expression between patients of different gender,age and course of disease(P>0.05).The expression of VDR mRNA and protein in peripheral blood lymphocytes of the observation group were lower than those of the control group,with significant difference between the two groups(P<0.05),there was no significant difference in CD8+level between the two groups(P>0.05).VDR receptor was negatively correlated with microRNA-155,positively correlated with CD4+and CD4+/CD8+(P<0.05),but not with CD8+(P>0.05).Conclusion The decreased expression of VDR has some effect on the immune response of PBC patients,considering that it is involved in the occurrence and development of PBC.
