微创颅内血肿穿刺引流术治疗高血压脑出血的最佳手术时间窗探讨 被引量:4

Discussion on optimal operation time window of minimally invasive intracranial hematoma puncture and drainage for hypertensive cerebral hemorrhage

摘要 目的采用微创颅内血肿穿刺引流术分析高血压脑出血(HICH)的最佳手术时间。方法回顾性分析义马煤业集团股份有限公司总医院神经外科2017年1月—2019年2月收治的100例HICH患者临床资料,将发病6 h内采用微创颅内血肿穿刺引流术治疗的50例患者纳入观察组,将发病6~12 h内采用微创颅内血肿穿刺引流术治疗的50例患者纳入对照组。比较两组术前、术后14 d格拉斯哥昏迷评分(GCS)、神经动能评分(NIHSS)及脑源性神经营养因子(BDNF)水平。结果术后14 d,观察组NIHSS评分为(4.34±1.19)分,低于对照组(7.02±1.76)分,GCS评分为(12.90±3.41)分,高于对照组(9.24±1.61)分,差异有统计学意义(t=8.920,6.863,P=0.000);术后14 d,观察组BDNF水平为(3.11±0.52)ng/mL,高于对照组(2.41±0.37)ng/mL,差异有统计学意义(t=7.756,P=0.000)。结论微创颅内血肿穿刺引流术治疗HICH的最佳手术时间窗为6 h内,在该时间段更容易达到血肿清除的最佳效果。 Objective To analyze the optimal operation time of hypertensive cerebral hemorrhage(HICH)with minimally invasive intracranial hematoma puncture and drainage.Methods The clinical data of 100 patients with HICH who were treated in our Department of Neurosurgery from January 2017 to February 2019 were retrospectively analyzed,and 50 patients treated with minimally invasive intracranial hematoma puncture and drainage within 6 hours of onset were included in the observation group,50 patients treated with minimally invasive intracranial hematoma puncture and drainage within 6-12 hours of onset were included in the control group.Glasgow Coma Scale(GCS),National Institute of Health stroke scale(NIHSS)and brain-derived neurotrophic factor(BDNF)levels were compared between the two groups before and 14 days after operation.Results Fourteen days after operation,NIHSS score of observation group was(4.34±1.19)points,which was lower than(7.02±1.76)points of control group,the GCS score of the control group was(12.90±3.41)points,which was higher than(9.24±1.61)points of the control group,with statistical significant differences(t=8.920,6.863,P=0.000);Fourteen days after operation,the level of BDNF in the observation group was(3.11±0.52)ng/mL,which was higher than(2.41±0.37)ng/mL of the control group,with a statistical significant difference(t=7.756,P=0.000).Conclusion The optimal time window for minimally invasive intracranial hematoma puncture and drainage for the treatment of HICH is within 6 hours,during which the best effect of hematoma removal is easier to achieve.
