摘要
目的分析丹参酮联合异维A酸治疗Ⅲ/Ⅳ级痤疮对性激素水平及血清白介素-8(interleukin-8,IL-8)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)的影响。方法选择我院2020年1月~2023年1月收治的90例中重度痤疮患者为研究对象,随机抽签法分为观察组和对照组,各45例。对照组口服异维A酸胶囊每次10 mg,每天2次;观察组在对照组治疗基础上加用丹参酮胶囊每次1 g,每天3次。比较两组患者临床疗效、治疗前后痤疮症状评分、炎症因子(TNF-α、IL-8)水平、性激素[睾酮(testosterone,T)、孕酮(progesterone,P)、雌二酮(estradion,E2)]水平变化以及用药不良反应发生情况。结果观察组总有效率显著高于对照组(P<0.05);治疗后两组炎性丘疹、脓疱、囊肿、粉刺等症状评分均显著下降(P<0.05),观察组炎性丘疹、脓疱、囊肿、粉刺等症状评分低于对照组(P<0.05);治疗后两组TNF-α、IL-8水平均显著下降(P<0.05),且观察组TNF-α、IL-8水平低于对照组(P<0.05);治疗后两组T、P水平显著下降,E2水平明显升高(P<0.05),且观察组T、P水平低于对照组,E2水平高于对照组(P<0.05);两组用药不良反应发生率比较无明显差异(P>0.05)。结论丹参酮联合异维A酸治疗Ⅲ/Ⅳ级痤疮疗效显著,可明显改善粉刺、丘疹等临床症状,调节性激素及血清IL-8、TNF-α水平,且无明显不良反应发生,安全可靠。
Objective To analyze the effects of tanshinone combined with isotretinoin on sex hormone levels,serum interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α)in the treatment of acne Ⅲ/Ⅳ.Methods A total of 90 patients with moderate and severe acne admitted to our hospital from January 2020 to January 2023 were selected,and randomly divided into observation group and control group,with 45 cases in each group.Control group took isotretinoin capsules 10 mg each time,twice a day;Observation group was treated with tanshinone capsule 1 g each time,3 times a day on the basis of control group.The clinical efficacy,acne symptom score,levels of inflammatory factors(TNF-α,IL-8),sex hormones(testosterone(T),progesterone(P),estradion(E2))before and after treatment and the occurrence of adverse drug reactions were compared between the two groups.Results The total effective rate of observation group was significantly higher than that of control group(P<0.05).After treatment,the scores of inflammatory papules,pustules,cysts and acne were significantly decreased in both groups(P<0.05),and the scores of inflammatory papules,pustules,cysts and acne in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of TNF-α and IL-8 in 2 groups were significantly decreased(P<0.05),and the levels of TNF-αand IL-8 in observation group were lower than those in control group(P<0.05).After treatment,T and P levels were significantly decreased,E2 levels were significantly increased(P<0.05),and T and P levels in observation group were lower than control group,E2 levels were higher than control group(P<0.05);There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Tanshinone combined with isotretinoin has a significant effect on the treatment of grade Ⅲ/Ⅳ acne,which can significantly improve the clinical symptoms such as acne and papular,regulate the levels of sex hormones,serum IL-8 and TNF-α,and has no obvious adverse reactions.It is safe and reliable.
