摘要
目的用尿碘/肌酐比值法评估哈尔滨市妊娠妇女不同孕期碘营养状况并了解妊娠妇女碘营养水平与甲状腺功能间的关系。方法选取2018年9月~2020年7月在哈尔滨市某大型三甲医院进行产检的妊娠妇女1966例。采用西门子公司化学分析仪检测其血清促甲状腺激素(TSH)、血清游离三碘甲状腺原氨酸(FT3)、血清游离甲状腺素(FT4)、甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(TgAb)。尿碘肌酐比值用中位数、四分位数间距M(P25,P75)表示。结果1996例妊娠妇女尿碘肌酐比值(UIC/Cr)中位数为140.57(98.78,210.75)μg/g,孕中期和孕晚期UIC/Cr中位数与孕早期相比差异具有统计学意义(P<0.05)。妊娠妇女不同孕期甲状腺异常率不同,差异具有统计学意义(P<0.05)。妊娠期甲状腺疾病患病率为26.9%,其中亚临床甲减和亚临床甲亢患病率较高,分别为6.23%和6.54%。比较妊娠妇女不同碘营养状况下甲状腺患病情况,差异不具有统计学意义(P>0.05),不同碘营养状态下甲状腺过氧化物酶抗体(TPOAb)水平有差异,差异具有统计学意义(P<0.05)。结论哈尔滨市妊娠妇女基本处于碘适宜水平,仍需密切关注妊娠妇女人群的碘营养状况,重视妊娠各期甲状腺功能监测。
Objective To evaluate the iodine nutrition status of pregnant women during different trimesters in Harbin by using the urine iodine/creatinine ratio method and to understand the relationship between iodine nutrition level and thyroid function in pregnant women.Methods A total of 1966 pregnant women who underwent antenatal examination were recruited from September 2018 to July 2020 in a large third-grade hospital in Harbin city.The serum levels of TSH,FT3,FT4,TPOAb and TgAb were measured by chemical analyzer of siemens.Urinary iodine creatinine ratio(UIC/Cr)were expressed as median and interquartile M(P25,P75).Results The median UIC/Cr of the 1996 pregnant women was 140.57(98.78,210.75)μg/g,Median UIC/Cr in the second and third trimesters of pregnancy was significantly different from that in the first trimester(P<0.05).Thyroid abnormalities in pregnant women were different among trimesters,and the difference was statistically significant.The prevalence of thyroid diseases during pregnancy was 26.9%,and the prevalence of subclinical hypothyroidism and subclinical hyperthyroidism were 6.23%and 6.54%,respectively.There was no statistically significant difference in the thyroid abnormal rate between pregnant women with different iodine nutrition status(P>0.05),while there was a statistically significant difference in thyroid peroxidase antibody(TPOAb)between pregnant women with different iodine nutrition status(P<0.05).Conclusion Pregnant women in Harbin are basically at the appropriate iodine level.We still need to keep a close watch on iodine nutrition status of pregnant women,attach great importance to the thyroid function monitoring of different gestation periods.
