摘要
目的:探讨铋剂四联法联合柴胡疏肝散治疗胃溃疡的疗效。方法:选取在江南大学附属医院消化内科接受治疗的136例胃溃疡患者为研究对象,随机为对照组(n=68)和观察组(n=68)。对照组采用铋剂四联疗法治疗(奥美拉唑+阿莫西林+克拉霉素+枸橼酸铋钾),观察组在对照组基础上加用柴胡疏肝散治疗。对比两组疗效、幽门螺杆菌根除率、治疗前后中医证候积分、血清学指标、6个月和12个月复发率以及幽门螺杆菌感染率。结果:观察组的总有效率为91.2%,明显高于对照组的总有效率(64.7%)。观察组的幽门螺杆菌根除率为92.6%,明显高于对照组的70.6%。治疗后,两组的胃脘胀闷、脘痛连胁、胸闷、嗳气、反酸的中医证候积分均明显低于治疗前,且观察组低于对照组。治疗后,两组的胃泌素及TNF-α均明显低于治疗前,且观察组低于对照组;两组的EGF均明显高于治疗前,且观察组高于对照组。随访6个月时,观察组的复发率及幽门螺杆菌感染率低于对照组。随访12个月时,观察组的复发率及幽门螺杆菌感染率均明显低于对照组。结论:铋剂四联法联合柴胡疏肝散治疗胃溃疡临床疗效明确,可降低中医症候积分,改善血清学指标,降低复发率及幽门螺杆菌感染率。
Objective To investigate the effect of sputum quadruple method combined with Chaihu Shugan Powder in the treatment of gastric ulcer.Methods A total of 136 patients with gastric ulcer admitted to Affiliated Hospital of Jiangnan University from March 2016 to September 2017 were divided into control group(n=68)and observation group(n=68).The control group was treated with expectorant quadruple therapy(omeprazole+amoxicillin+clarithromycin+bismuth potassium citrate).The observation group was treated with quadruple therapy combined with Chaihu Shugan Powder.The efficacy of the two groups,the eradication rate of Helicobacter pylori,the TCM syndrome score before and after treatment,the serological index,the recurrence rate and the Helicobacter pylori infection rate of 6 months and 12 months were compared.Results The effective rate of the observation group was higher than the control group(91.2%vs 64.7%).The Helicobacter pylori eradication rate of the observation group was higher than the control group(92.6%vs 70.6%).After treatment,the scores of TCM syndromes of stomach cramps,sputum pain,chest pain,suffocation,and acid reflux were significantly lower than those before treatment,and the observation group was lower than the control group.After treatment,the gastrin and TNF-αin the two groups were lower than before the treatment,and the observation group was lower than the control group;the EGF of the two groups were significantly higher than before the treatment,and the observation group was higher than the control group.At 6 months follow-up,the recurrence rate and Helicobacter pylori infection rate in the observation group were lower than those in the control group.At 12 months follow-up,the recurrence rate and Helicobacter pylori infection rate in the observation group were lower than the control group.Conclusion The curative effect of sputum quadruple combined with Chaihu Shugan Powder in the treatment of gastric ulcer is clear,which can reduce TCM syndrome scores,improve serum markers,reduce recurrence rate and Helicobacter pylori infection rate.
