摘要
目的分析"十三五"期间(2016-2020年)昆山市重点人群碘营养水平,为制定"十四五"期间昆山市碘缺乏病防治策略提供科学依据。方法按照《江苏省碘缺乏病监测方案》,每年根据地理方位采用分层随机抽样法抽取8~10岁儿童和孕妇,采集其尿样和家中食用盐样,测定尿碘及盐碘含量。结果2016-2020年昆山市共采集1614份盐样,其中碘盐覆盖率为96.53%,碘盐合格率为99.81%,合格碘盐食用率为96.34%。共调查8~10岁儿童尿样1089份,尿碘中位数202.0μg/L,男童和女童尿碘中位数分别为217.0μg/L和188.0μg/L,性别间差异有统计学意义(χ^(2)=16.009,P<0.05)。共调查孕妇尿样525份,尿碘中位数154.0μg/L,其中尿碘值<150μg/L的占比47.8%,各年份间孕妇尿碘含量中位数差异无统计学意义(χ^(2)=4.188,P>0.05)。结论"十三五"期间昆山市重点人群总体碘营养水平处于适宜水平,但孕妇碘营养不足人数比例较高,存在碘缺乏风险。"十四五"期间需进一步加强重点人群碘营养监测及动态评估,针对不同人群精准化碘营养教育。
Objective The iodine nutrition level of key population in Kunshan during the 13th Five Year Plan(2016-2020)was analyzed,which provided scientific basis for the formulation of the prevention and control policies of iodine deficiency disease in the 14th Five Year Plan of the region.Methods Iodine nutrition monitoring was carried out for five consecutive years according to the Monitoring Program of iodine Deficiency Disorders in Jiangsu province.Children aged 8-10 years and pregnant women were randomly selected according to geographical location every year,and their urine samples and edible salt samples were collected to determine the content of urine iodine and salt iodine.Results A total of 1614 salt samples were collected in Kunshan city from 2016 to 2020.The coverage rate of iodized salt was 96.53%,the qualified rate of iodized salt was 99.81%,and the edible rate of qualified iodized salt was 96.34%.A total of 1089 urine samples were collected from children aged 8-10 years,and the median urine iodine was 202.0μg/L.There was significant difference in the median urine iodine content between different genders(χ^(2)=16.009,P<0.05).The content of urine iodine in male was higher than that in female.A total of 525 urine samples of pregnant women were collected,and the median of urine iodine was 154.0μg/L.The urine iodine value less than 150μg/L accounted for 47.8%.There was no significant difference in the median urine iodine content among different years(χ^(2)=4.188,P=0.381).Conclusion During the 13th Five Year Plan,the overall iodine nutrition level of key population in Kunshan city is at an appropriate level,but the proportion of pregnant women with iodine deficiency is high,and there is a risk of iodine deficiency.During the period of the 14th Five Year Plan,it is necessary to further strengthen the monitoring and dynamic assessment of iodine nutrition of key groups,and to provide accurate iodine nutrition education for different groups.
