摘要
目的探究百令胶囊联合羟苯磺酸钙胶囊治疗糖尿病肾病(diabetic nephropathy,DN)的临床效果及对患者血清α-klotho蛋白、几丁质酶3样蛋白1(chitinase 3-like protein 1,YKL-40)、内脏脂肪组织特异性丝氨酸蛋白酶抑制物(visceral adipose-specific serine protease inhibitor,Vaspin)水平的影响。方法本研究选取2019-03/2020-03月在作者医院收治的90例DN患者,采用随机数字表法分为对照组和研究组,各45例。对照组在常规治疗的基础上给予百令胶囊,每次2.0 g,1次/d;研究组在对照组基础上,联合羟苯磺酸钙胶囊,每次0.5 g,3次/d。两组均连续用药3个月。结果治疗前两组血肌酐(serum creatinine,Scr)、胱抑素C(cystatin C,CysC)、血尿素氮(blood urea nitrogen,BUN)水平比较,差异无统计学意义(P>0.05);治疗后两组Scr、CysC、BUN水平均低于治疗前,且研究组低于对照组(P<0.05)。治疗前两组血清α-klotho蛋白、YKL-40、Vaspin水平比较差异无统计学意义(P>0.05);治疗后两组血清α-klotho蛋白均降低,YKL-40、Vaspin水平均升高,且研究组α-klotho蛋白水平低于对照组,YKL-40、Vaspin水平高于对照组(P<0.05)。研究组总有效率明显高于对照组(91.11%vs.66.67%,χ^(2)=8.073,P<0.05)。结论百令胶囊联合羟苯磺酸钙胶囊治疗DN临床疗效好,对血清α-klotho蛋白、YKL-40、Vaspin的表达有促进作用,具有一定临床应用。
Objective To investigate the clinical effect of Bailing capsule combined with calcium dobesilate capsule in the treatment of diabetic nephropathy(DN)and the influence of serumα-klotho protein,chitinase 3-like protein 1(YKL-40)and visceral adipose-specific serine protease inhibitor(Vaspin)levels.Methods In this study,90 patients with DN admitted to author′s hospital from March 2019 to March 2020 were selected and divided into control group and study group by random number table method,with 45 cases in each group.Control group was given Bailing capsule,2.0 g each time once a day,in addition to conventional treatment.The study group was combined with calcium dobesilate capsule on the basis of the control group,0.5 g each time 3 times a day.Both groups were treated continuously for 3 months.Results There was no significant difference in serum creatinine(Scr),cystatin C(CysC)and blood urea nitrogen(BUN)levels between the 2 groups before treatment(P>0.05).After treatment,the levels of Scr,CysC and BUN in 2 groups were lower than before,and the study group were lower than those of the control group(P<0.05).There was no significant difference in serumα-klotho protein,YKL-40 and Vaspin levels between the 2 groups before treatment(P>0.05).After treatment,serumα-klotho protein was decreased in both 2 groups,and the levels of YKL-40 and Vaspin were increased.Theα-klotho protein level in the study group was lower than that in the control group,while the levels of YKL-40 and Vaspin were higher than those in the control group(P<0.05).The total effective rate in the study group was higher than that in the control group(91.11%vs.66.67%,χ^(2)=8.073,P<0.05).Conclusion Bailing capsule combined with calcium dobesilate capsule has good clinical efficacy in the treatment of DN,and can promote the expression ofα-klotho protein,YKL-40 and Vaspin in serum,which has certain clinical application.
