摘要
目的通过数字化骨盆三维重建模型进行数字化测量,研究正常成人髂嵴弧度,比较同性别患者左、右侧髂嵴弧度及不同性别患者髂嵴弧度的差异,为临床提高髂骨骨折复位质量提供解剖学参数支持。方法采集2016-01/2018-12月期间于作者医院行电子计算机断层扫描(computed tomography,CT)三维重建检查的正常成人完整骨盆数据,男60例(120侧),女40例(80侧),平均年龄42.18岁。通过Mimics 20.0软件和Geomagic Studio 12软件对骨盆CT数据进行三维重建、切割标记和投影,对正常成人髂嵴弧度进行测量。结果全髂嵴弧度,男(3.08±0.13)rad;女(2.92±0.13)rad;前中部髂嵴内唇弧度,男(2.28±0.17)rad,女(2.15±0.14)rad;后部髂嵴外唇弧度,男(1.39±0.28)rad,女(1.18±0.31)rad。同性别患者左、右侧髂嵴弧度比较差异无统计学意义(P>0.05)。全髂嵴弧度、前中部髂嵴内唇弧度与后部髂嵴外唇弧度,两性之间(左右侧混合)差别均有统计学意义(P均<0.05);三者均为男性大于女性。结论髂嵴弧度在不同性别间差异明显,该组解剖学参数可为术中髂骨骨折的解剖复位提供参考,并能评价骨折旋转移位情况,弥补Matta评价标准的不足。
Objective To obtain the iliac crest radian in normal adult,by three-dimensional(3 D)computed tomography(CT)reconstruction and digital measurement of adult intact pelvis,comparing the left and right iliac crests of patients of the same gender and the differences of the iliac crests of patients of different genders,and providing anatomical parameter support for clinically improving the quality of ilium fracture reduction.Methods Complete adult pelvic data of CT 3 D reconstruction examinations were collected from authors′hospital from January 2016 to December 2018.The subjects consisted of 60 males(120 sides),40 females(80 sides),with an average of 42.18 years old.Importing data into Mimics 20.0 and Geomagic Studio 12 software in DICOM(Digital Imaging and Communications in Medicine)and.STL format,cutting,masking and projecting the data,then measuring the iliac crest radian of normal adults.Results The iliac crest radian of normal adults between male and female were(3.08±0.13)rad versus(2.92±0.13)rad at the full part,while(2.28±0.17)rad versus(2.15±0.14)rad at the anterior central inner part,and(1.39±0.28)rad versus(1.18±0.31)rad at the posterior external part.There was no significant differences between left and right parts of patients of the same gender(P>0.05).The total iliac crest radian,anterior central iliac crest radian and posterior iliac crest radian,and the difference between the different genders(mixed left and right)were significantly different(all P<0.05),all three were male higher than female(all P<0.05).Conclusion Adult iliac crest radian is significantly different between men and women.This group of anatomical parameters can provide a reference for anatomical reduction of the iliac fracture during operation,and can evaluate the rotation displacement of the fracture,compensating the deficiency of Matta criterion.
