甲状腺手术中应用精细化被膜解剖法对预防喉返神经与甲状旁腺损伤的效果观察 被引量:4

Observation of the Effect of Fine Capsule Anatomy on Preventing the Injury of Recurrent Laryngeal Nerve and Parathyroid Gland in Thyroid Surgery

摘要 目的分析甲状腺手术中应用精细被膜解剖法预防喉返神经与甲状旁腺损伤的效果。方法以2019年1月~2020年12月收治的甲状腺手术者为研究对象,共72例,按照手术方案不同,等分为对照和观察组。对照组给予传统手术,观察组给予精细被膜解剖法。对比分析两组患者喉返神经与甲状旁腺损伤情况。结果观察组喉返神经损伤率、甲状旁腺损伤率均低于对照组;术后1 d、7 d,观察组血钙水平、甲状旁腺激素水平均高于对照组,差异有统计学意义(P<0.05);但术前、术后1个月两个阶段中,患者两项指标水平基本相同,差异无统计学意义(P>0.05)。结论对甲状腺手术患者给予精细被膜解剖法,可最大限度降低对患者的喉返神经、状旁腺造成的损伤现象,具有显著的应用优势。 Objective To analyze the effect of fine capsule anatomy in preventing the injury of the recurrent laryngeal nerve and parathyroid gland during thyroid surgery.Methods A total of 72 patients who had undergone thyroid surgery from January 2019 to December 2020 were selected as the research object.According to different surgical protocols,they were equally divided into control and observation groups.The control group was given traditional surgery,and the observation group was given the fine capsule anatomy.The injury of the recurrent laryngeal nerve and parathyroid glands of the two groups of patients was compared and analyzed.Results The recurrent laryngeal nerve injury rate and parathyroid injury rate in the observation group were lower than those in the control group;on the 1 st and 7 th day after operation,the blood calcium level and parathyroid hormone level of the observation group were higher than those of the control group,the difference was statistically significant(P<0.05);but in the preoperative and one month postoperative stages,the levels of the two indicators of the patients were basically the same,and the difference was not statistically significant(P>0.05).Conclusions The fine capsule anatomy for patients undergoing thyroid surgery can be Minimize the damage to the patient′s recurrent laryngeal nerve and parathyroid gland,which has significant application advantages.
