健康人群尿液中邻苯二甲酸酯代谢物与肺功能关联的随机交叉试验研究 被引量:2

Randomized crossover study on association between urine phthalate metabolites and lung function among healthy subjects

摘要 目的探讨尿液中邻苯二甲酸酯(phthalic acid esters,PAEs)代谢物浓度与健康人群肺功能间的关联。方法采用随机交叉研究设计,将符合纳入标准的北京某高校46名健康大学生随机分为两组,分别进行为期1周的空气净化器干预试验(含高效滤芯的真空气净化器及不含高效滤芯的假空气净化器的各一组),间隔2周洗脱期后,两组干预方式互换再干预1周。每次随访阶段结束后收集研究对象的晨尿,测定其中PAEs代谢物浓度,并对研究对象的肺功能进行测定,将两次随访数据集整理合并后纳入线性混合效应模型分析尿液中PAEs代谢物与其肺功能变化间的关联。结果研究对象尿液中邻苯二甲酸单苄酯(mono-benzyl phthalate,MBzP)浓度每升高一个四分位间距(interquartile range,IQR),呼气峰流速(peak expiratory flow,PEF)降低2.47%(95%CI:0.18%~4.70%)。在校正其它污染物时该关联稳定。结论尿液中PAEs代谢物浓度升高与健康人群肺功能降低存在关联,提示PAEs暴露可能对健康人群肺功能产生影响。 Objective To estimate the association of urine phthalic acid esters(PAEs)with lung function of healthy subjects.Methods A randomized crossover trial was conducted among 46 healthy students in a university in Beijing who met the inclusion criteria.These subjects received true or sham indoor air purifiers intervention in random order,with a 2-week washout period between the two visits.Urine phthalate metabolites and lung function of subjects were measured after each stage of intervention.The data from two urine collections and lung function tests were mixed.Linear mixed-effect models were applied to evaluate the associations of urine phthalate metabolites and lung function.Results The results of the linear mixedeffects model showed that for each IQR increase in mono-benzyl phthalate(MBzP)in the urine of the observed subjects,peak expiratory flow(PEF)decreased by 2.47%(95%CI:0.18%-4.70%).The association still existed after adjusting for other pollutants.Conclusion The increase of urine PAEs metabolites is associated with the decrease of lung function of healthy subjects,it indicates that PAEs exposure may affact the lung function of healthy subjects.
