摘要
目的分析探讨宫颈上皮内瘤变(CIN)级别与感染不同HR-HPV亚型的相关性。方法以我院妇产科于2018年2月~2019年2月期间所接收的行TCT细胞学检查及HPV-DNA分型检测的600例患者作为此次研究对象,辅助以阴道镜行多点活检病理检查共诊断出520例CIN患者,然后对CIN患者感染HPV状况及CIN级别与感染不同HRHPV亚型的相关性予以分析与总结。结果520例CIN患者中包括348例CIN 1患者,相应占比为33.08%,HPV的感染率为86.05%,共检出21种HPV亚型,其中以HPV16、HPV58、HPV52、HPV33及HPV31占比最高;172例CIN2、3患者,相应占比为66.92%,HPV感染率为90.80%,共检出19种HPV亚型,其中以HPV16、HPV58、HPV33、HPV52及HPV31占比最高;不同CIN级别患者中HPV16与HPV52型检出率比较具有统计学意义(P<0.05),而其余亚型于CIN不同级别患者中的检出率比较差异不具有统计学意义(P>0.05);Logistic回归分析表明,HPV16、HPV33与患者CIN级别呈正相关性,而HPV52、HPV59与CIN级别呈负相关性。结论CIN病变级别与HR-HPV感染亚型存在密切的关系,而HPV16、HPV52与CIN病变级别呈正相关性,且HPV16、HPV52亚型感染是患者出现高瘤变的危险因素。
Objective To investigate the correlation between the grade of cervical intraepithelial neoplasia(CIN)and different subtypes of HR-HPV infection.Methods A total of 520 CIN patients were diagnosed by colposcopy and multi-point biopsy.The relationship between the status of HPV infection in CIN patients and the level of CIN and different subtypes of HR-HPV infection was analyzed and summarized.Results Among 520 patients with CIN,348 were CIN-1 patients,the corresponding proportion was 66.92%,and the infection rate of HPV was 86.21%.Twenty-one HPV subtypes were detected,of which HPV16,HPV58,HPV52,HPV33 and HPV31 were the highest;172 patients with CIN2 and 3 accounted for 33.08%,and the infection rate of HPV was 90.70%.Nineteen HPV subtypes were detected,of which HPV16,HPV58,HPV33,HPV52 and HPV31 accounted for the highest proportion.The detection rates of HPV16 and HPV52 in grade patients were statistically significant(P<0.05),while the detection rates of other subtypes in different grades of CIN patients were not significantly different(P>0.05);Logistic regression analysis showed that HPV16 and HPV33 were positively correlated with CIN level,while HPV52 and HPV59 were negatively correlated with CIN level.Conclusion There is a close relationship between CIN lesion grade and HR-HPV infection,while HPV16,HPV52 and CIN lesion grade are positively correlated,and HPV16,HPV52 subtype infection is a risk factor for patients with high tumorigenesis.
