体质量对2型糖尿病患者神经病变的影响 被引量:1

Effect of body mass on neuropathy in patients with type 2 diabetes mellitus

摘要 目的分析体质量对2型糖尿病(T2DM)患者神经病变的影响。方法收集2016年1月至2017年1月郑州人民医院651例T2DM患者,测量各种临床指标,分析体质量对神经病变的影响。结果体质指数(BMI)减少(r=0.174,P<0.01)、最大体质量距今时间(r=0.207,P=0.000)和自律神经病变呈正相关且差异有统计学意义,当前BMI(r=-0.178,P<0.01)、最大BMI(r=-0.076,P=0.068)和自律神经病变呈负相关,其中当前BMI比较差异有统计学意义(P<0.05),最大BMI比较差异未见统计学意义(P>0.05);BMI减少(r=0.119,P=0.005)、最大体质量距今时间(r=0.178,P=0.000)、最大BMI(r=0.026,P=0.535)和末梢神经病变呈正相关,除最大BMI外比较差异均有统计学意义,当前BMI(r=-0.049,P=0.247)和末梢神经呈负相关,差异未见统计学意义(P>0.05)。Binary logistic回归提示病程(OR=1.146,P=0.010)和空腹血糖(OR=1.145,P=0.060)是自律神经的独立危险因素,病程(OR=1.273,P=0.000)和BMI下降(OR=2.644,P=0.030)是末梢神经的独立危险因素。结论2型糖尿病患者体质量下降越大,发生末梢神经病变的风险越大。 Objective To analyze the effect of body mass on neuropathy in type 2 diabetes mellitus(T2 DM).Methods A total of 651 T2 DM patients from Zhengzhou People’s Hospital from January 2016 to January 2017 were collected.The influence of body mass on neuropathy was analyzed.Results The decrease of BMI(r=0.174,P=0.000),the time from the most gross mass(r=0.207,P=0.000)were positively correlated with the autonomic neuropathy.The current BMI(r=-0.178,P=0.000),the maximum BMI(r=-0.076,P=0.068)were negatively correlated with the autonomic neuropathy,among which the current BMI difference was significant,the maximum BMI difference was not significant;the decrease of BMI(r=0.000)119,P=0.005),the time from the most gross mass(r=0.178,P=0.000),the maximum BMI(r=0.026,P=0.535)were positively correlated with peripheral neuropathy.Except for the maximum BMI,the difference was significant.At present,BMI(r=-0.049,P=0.247)were negatively correlated with peripheral neuropathy,but the difference was not significant.Binary logistic regression showed that course of disease(OR=1.146,P=0.010)and fasting blood glucose(OR=1.145,P=0.060)were independent risk factors of autonomic nervous system,and course of disease(OR=1.273,P=0.000)and decrease of BMI(OR=2.644,P=0.030)were independent risk factors of peripheral nerve.Conclusions The greater the decrease of body mass,the greater the risk of peripheral neuropathy.
