摘要
目的探讨多发颅内外动脉狭窄对急性轻型卒中1年复发的影响。方法选取2014年1月至2018年11月任丘康济新图医院神经内科急性轻型卒中患者2390例,采用多因素logistic回归方程分析多发脑血管狭窄对轻型卒中患者1年复发的影响。结果2390例患者中男886例,女1504例,年龄23~95岁,平均(62.6±11.1)岁。1年内卒中复发70例,复发率为2.93%。颅内外动脉狭窄发生率为41.05%,其中多发颅内外动脉狭窄20.33%。多因素回归分析结果显示,同型半胱氨酸(homocysteine,Hcy)(OR=1.015,95%CI:1.006~1.024,P=0.001)越高、多发颅内外动脉狭窄(OR=3.292,95%CI:1.925~5.631,P=0.000)的患者1年卒中复发风险越高。结论高Hcy血症和多发颅内外动脉狭窄是轻型卒中1年复发风险的独立危险因素。
Objective To explore the effect of multiple intracranial and extracranial artery stenosis on the risk of one-year recurrence of acute mild stroke.Methods A total of 2390 patients with acute mild stroke admitted to the Department of Neurology,Renqiu Kangjixintu Hospital from January 2014 to November 2018 were selected.Multivariate logistic regression equation was used to analyze the effect of multiple intracranial and extracranial artery stenosis on the risk of one-year recurrence of acute mild stroke.Results Among the 2390 patients,there were 886 males and 1504 females,aged from 23 to 95 years,with an average age of(62.6±11.1)years.A total of 70 cases of stroke recurred within one year,and the recurrence rate was 2.93%.The incidence of intracranial and extracranial artery stenosis was 41.05%,of which 20.33%were multiple intracranial and extracranial artery stenosis.Multivariate logistic regression analysis results showed that patients with higher Hcy(OR=1.015,95%CI:1.006-1.024,P=0.001)and multiple intracranial and extracranial artery stenosis(OR=3.292,95%CI:1.925-5.631,P=0.000)had higher one-year stroke recurrence risk.Conclusions Hyperhomocysteinemia and multiple intracranial and extracranial artery stenosis are independent risk factors for one-year recurrence of minor stroke.
