摘要
目的:探讨甲状腺过氧化物酶抗体(TPOAb)对早孕妇女甲状腺功能和糖代谢水平的影响。方法:纳入2016年4月至2018年4月本院就诊早孕妇女160例,收集就诊时TPOAb水平,检测甲状腺功能指标[促甲状腺素(TSH)、游离三碘甲腺原氨酸(FT3)及游离甲状腺素(FT4)]和糖代谢指标[空腹血糖(FPG)、餐后2 h血糖(PBG)及胰岛素抵抗指数(HOMA-IR)],跟踪随访妊娠结局。分析TPOAb与甲状腺功能和糖代谢水平的关系。结果:TPOAb阳性组妇女FT3和FT4水平显著低于TPOAb阴性组,TSH和PBG显著高于TPOAb阴性组,差异均有统计学意义(均P<0.05)。TPOAb与FPG、HOMA-IR、TSH呈正相关,与FT4呈负相关(均P<0.05)。TPOAb阳性组不良妊娠发生率显著高于TPOAb阴性组(P<0.05)。结论:早孕妇女TPOAb水平与甲状腺功能和糖代谢状态关系密切,TPOAb阳性可增加不良妊娠结局风险。
Objective:To investigate the impact of thyroid peroxidase antibody(TPOAb)level on thyroid function and glucose metabolism in early pregnancy.Methods:Included in this study were 160 women during early pregnancy who visited our hospital between April 2016 and April 2018.The subjects were recoded for TPOAb levels,and measured for thyroid function indicators[thyrotropin(TSH),free triiodothyronine(FT3)and free tetraiodothyronine(FT4)]and glycometabolism[fasting blood glucose(FPG),postprandial 2-hour blood glucose(PBG)and the homeostasis model assessment of insulin resistance(HOMA-IR)]at the time of hospital visit.The pregnancy outcome was followed up.The relationships between TPOAb and thyroid function and glucose metabolism were analyzed.Results:The women with positive TPOAb presented significantly lower levels of FT3 and FT4,and higher levels of TSH and PBG,compared with TPOAb-negative women,with statistically significant differences(allP<0.05).The TPOAb levels were positively correlated with FPG,HOMA-IR and TSH,and negatively correlated with FT4(allP<0.05).The incidence of adverse pregnancy outcome in TPOAb positive group was significantly higher than that in TPOAb negative group(P<0.05).Conclusion:The level of TPOAb in early pregnant women is closely associated with thyroid function and glucose metabolism.Women with positive TPOAb may face higher risk of adverse pregnancy outcome.
