摘要
目的探讨新型冠状病毒肺炎疫情期间在重症监护室(ICU)实行"云探视"的应用及效果。方法选取2019年11月1日―2020年3月31日在ICU住院救治的382名患者作为研究对象,其中2019年11月1日―2020年1月17日住院的患者为对照组,共252例,采用床边预约式探视模式;2020年1月18日―2020年3月31日住院的患者为试验组,共130例,采用"云探视"模式。比较两组患者在ICU治疗期间院内感染发生情况、谵妄发生率、患者家属焦虑程度、患者及家属对护理工作满意度、医生护士对两种探视模式满意度。结果试验组患者呼吸机相关性肺炎率、留置导尿管相关性感染率、护理投诉率均低于对照组(p<0.05);试验组患者及家属对护理满意度、表扬率、医护人员满意度均高于对照组(p<0.05);两组患者谵妄发生率、患者家属焦虑程度无统计学差异(p>0.05)。结论"云探视"有利于新型冠状病毒肺炎的防控、降低ICU患者相关性感染,缓和医患矛盾,提高了患者家属对护理工作的满意度。
Objective To explore the application and effect of"cloud visits"in the intensive care unit(ICU)during COVID-19 epidemic.Methods A total of 382 patients who were hospitalized in the ICU from November 1,2019 to March 31,2020 were selected as the study subjects.Among them,the patients hospitalized from November 1,2019 to January 17,2020 were selected as the control group,that is a total of 252 cases.The bedside appointment visit mode was adopted.The patients hospitalized from January 18,2020 to March 31,2020 were the experimental group,a total of 130 cases,and the"cloud visit"mode was adopted.The incidence of nosocomial infections,the incidence of delirium,the degree of anxiety of the patients’family members,the satisfaction of the patients and their families with nursing work,and the satisfaction of doctors and nurses with the two visiting modes during the ICU treatment were compared between the two groups.Results The rates of ventilator-associated pneumonia,indwelling catheter-related infections,and nursing complaints in the experimental group were lower than those in the control group(p<0.05);The satisfaction rate,praise rate with nursing care of patients in the experimental group and their families,as well as the satisfaction rate of medical staff,were higher than those of the control group(p<0.05);There was no significant difference in the incidence of delirium and the degree of anxiety of the patients’family members between the two groups(p>0.05).Conclusion"Cloud visit"is beneficial to the prevention and control of COVID-19,reduce the related infections of ICU patients,ease the contradiction between doctors and patients,and improve the satisfaction of patients’families with nursing work.
