低管电压技术对甲状腺CT值的影响

摘要 目的:探讨低管电压CT扫描对甲状腺实质CT值的影响。方法:连续收集2017年12月至2018年2月间行颈部或颈椎CT扫描的患者40例,按时间先后分成A(20例)、B(20例)两组,对应管电压分别为100 Kvp、80 Kvp。两组均与自身对照(管电压为120 Kvp,对照组图像为同次胸部扫描或前后1个月内包含甲状腺CT图像),比较平扫及增强扫描甲状腺左右叶及胸锁乳突肌CT值,比较A、B两组甲状腺实质强化幅度差ΔA和ΔB。结果:A、B两组平扫甲状腺左、右叶CT值均高于自身对照组,且差异有统计学意义(P<0.05);A、B组甲状腺左、右叶增强CT值均高于自身对照组,A组无统计学差异(P>0.05),但B组有统计学差异(P<0.05);A、B两组甲状腺实质强化幅度差具有统计学意义(P<0.05)。A、B组胸锁乳突肌CT值与对照组比较均无统计学差异(P> 0.05)。结论:低管电压技术在甲状腺CT平扫及增强扫描中均影响甲状腺CT值,管电压分别为120 Kvp、 100 Kvp、80 Kvp时,对应甲状腺CT值依次增加。 Objective:To discuss the effect of CT scan with low tube voltage(TV)on CT value of thyroid parenchyma.Methods:40 consecutive patients underwent neck or cervical vertebra CT scan between December 2017 to February 2018 were collected, which were divided into two groups based on tube voltage in chronological order: group A(n=20)with TV 100 Kvp and group B(n=20)with TV 80 Kvp.A self-contrast analytical method was used in both groups,compared with the including thyroid images scaned with tube voltage 120 Kvp at the same time or in one month. The CT value of thyroid parenchyma and sternocleidomastoid were measured on pre-and post-enhanced images,and compared with self-contrast group,respectively.The enhanced degree deviation of thyroid parenchyma to compared group in group A(ΔA)and that in group B(ΔB)were also calculated. Results:The CT values of thyroid parenchyma in both group A and B were higher than those in control group on pre-contrast images,which has significant statistical differences(P<0.05);similarly on post-enhanced images,there was significantdifference between group B and self-contrast group(P<0.05),which was different from group A(P>0.05).The difference between ΔA and ΔB indicated statistical significance(P<0.05).There were no significant difference between CT value of sternocleidomastoid in both groups and their self-contrast group whether pre-and post-enhanced scan(P> 0.05). Conclusion:The CT value of thyroid parenchyma was higher on either plain or enhanced scan with low tube voltage(100 Kvp 、80 Kvp) than that with conventional tube voltage(120 Kvp),and adding value was much more with 80 Kvp than 100 Kvp.
