摘要
目的:调查分析炎症性肠病(IBD)患者的饮食不耐受及饮食限制情况,以期为IBD患者饮食指导提供参考。方法:以2020年2月至2021年2月于我院就诊的95例IBD患者为研究对象,其中克罗恩病(CD)63例,溃疡性结肠炎(UC)32例。应用自制饮食调查表调查患者饮食不耐受及饮食限制情况,以及获取饮食限制信息的主要渠道。结果:1)饮食不耐受情况:CD患者中41例(65.08%)至少有1种饮食不耐受,29例(46.03%)不耐受饮食≥3种;前3位不耐受饮食分别为乳制品、生冷食物、油腻食物。UC患者中25例(78.13%)至少有1种饮食不耐受,12例(37.5%)不耐受饮食≥3种;前3位不耐受饮食分别为生冷食物、辛辣食物、乳制品。2)饮食限制情况:CD患者中60例(95.24%)存在饮食限制现象,48例(76.19%)限制饮食≥3种;前3位限制饮食为辛辣食物、生冷食物、乳制品。UC患者中32例(100%)存在饮食限制现象,22例(68.75%)限制饮食≥3种;前3位限制饮食为生冷食物、辛辣食物、乳制品。3)饮食限制信息获取渠道:主要来自医护人员(73.22%)、自身经验(61.05%)、病友(41.89%)、网络(33.63%)、健康手册(31.58%)。结论:大多数IBD患者存在饮食不耐受情况,但存在个体差异性;IBD患者普遍存在过度饮食限制现象,这可能与患者对疾病的不正确认知及仅凭病友建议而限制某种饮食有关。临床医护人员可根据患者具体情况开展个性化饮食指导,以便更好地控制其临床症状。
Objective To investigate the status on diet intolerance and restriction of inflammatory bowel disease(IBD)patients,so as to provide the reference for diet induction of IBD patients.Methods Enrolled 95 IBD patients treated in authors’hospital(2020-02-2021-02),including 63 cases of CD and 32 of UC,into this research:by using author-self-developed diet scale to investigate IBD patients’diet intolerance and restriction status,as well as main pathway to obtain the information on diet restriction.Results 1)the status on diet intolerance:in 63 cases of CD,41(65.08%)had intolarance on at least one diet,29(46.03%)on≥3 diets,the former 3 orders of diet were dairy products,raw and cold food,and greasy food;in 32 cases of UC,25(78.16%)on one diet at least,12(37.50%),≥3 diets,the former 3 orders of ones were raw and cold food,pungent food,and diary products.2)the status on diet restriction:in the CD patients,60 cases(95.24%)had restriction on diet,48(76.19%),≥3 diets;the former 3 order of diets were pungnt food,raw and cold food,and dairy products;in the UC patients,32 cases(100%)had restriction on diet,22(68.75%),≥3 diets,in orders as raw and cold food,pungent food,and dairy products.3)the pathway to obtain the information on diet restriction:mainly,from medical staff(73.22%),self experience(61.05%),and wardmates(41.89%),as well as online cyber(33.63%)and health handbook(31.58%).Conclusion The most of IBD patients have diet intderance,but in personalized difference;IBD patients generally have hyperphagia which could be them don’t correctly know or only based on wardmate’s suggestion,then to restrict some food,therefore,staff should to take personalized diet induction for them according to their concrete condition,so that control clinical symptems better.
