摘要
目的:分析内镜下乳头括约肌小切开术(sEST)联合十二指肠乳头球囊扩张术(EPBD)治疗胆总管结石的临床疗效.方法:收集2018年3月至2021年3月本院收治的76例胆总管结石患者资料,根据手术方法不同分为研究组(n=39,sEST+EPBD手术治疗)和对照组[n=37,单纯内镜乳头括约肌切开术(EST)治疗].对比两组患者的治疗效果.结果:研究组手术总成功率、一次性成功率均明显高于对照组,机械碎石率低于对照组(P<0.05).两组术后血清直接胆红素(DBIL)、总胆红素(TBIL)水平明显低于术前(P<0.05),而术后两组间血清DBIL、TBIL水平比较无差异(P>0.05).研究组术后并发症发生率明显低于对照组(P<0.05).结论:sEST+EPBD治疗胆总管结石患者取石总成功率、一次性取石成功率均高,且安全有效.
Objective:To analyze the clinical efficacy of small endoscopie sphincterotomy(sEST)combined with endoscopic papillary balloon dilatation(EPBD)in the treatment of choledocholithiasis.Methods:The data of 76 patients with choledocholithiasis admitted to our hospital from March 2018 to March 2021 were collected,and according to different surgical methods,they were divided into a study group(n=39,sEST+EPBD surgery)and a control group[n=37,endoscopic sphincterotomy(EST)surgery].The treatment effect of the two groups of patients was compared.Results:The total success rate and one-time success rate of the study group were significantly higher than those of the control group,with a lower mechanical crushing rate than the control group(P<0.05).The levels of serum direct bilirubin(DBIL)and total bilirubin(TBIL)after the operation of the two groups were significantly lower than those before the operation(P<0.05),and there were no differences in the levels of DBIL and TBIL between the groups after the operation(P>0.05).The incidence of postoperative complications in the study group was significantly lower than that in the control group(P<0.05).Conclusion:sEST+EPBD treatment for patients with choledocholithiasis has a higher total success rate and one-time stone retrieval rate,and is safe and effective.
