摘要
目的探讨磁共振成像(magnetic resonance imaging,MRI)联合CTU尿路成像(computer tomography urography,CTU)在经尿道柱状水囊前列腺扩开术中应用价值.方法选择2018年1月至2019年1月遵义医科大学附属医院泌尿外科收治的前列腺增生症患者10例,年龄70~85岁,中位年龄72岁.排尿困难病史6~10年,中位数8年.因患者合并糖尿病,高血压,心功能不全等行经尿道前列腺电切术风险较大,均行经尿道前列腺柱状水囊扩开术.患者术前均行MRI检查,术后1年行排尿期CTU检查.结果10例患者术前MRI显示排尿期前列腺尿道段尿道狭窄,闭塞,术后1年CTU排尿期前列腺显示前列腺尿道宽敞,呈漏斗状,无术中,术后大出血并发症,术后1年均排尿通畅.结论MRI联合CTU可以有效评价前列腺增生症患者前列腺尿道形态改变,为临床扩张经尿道前列腺柱状水囊扩开术提供可靠的科学依据.术前行排尿期MRI也可排除前列腺癌,更加精准测量前列腺体积后指导选择球囊导管,避免医疗资源的浪费.
Objective To explore the clinical value of MRI combined with CTU in transurethral columnar balloon dilation of prostate(TUCBDP).Methods From January 2018 to January 2019,the clinical data of 10 benign prostatic hyperplasia(BPH)patients aged 70~85(median age:72)were selected.These patients had a history of dysuria for 6~10 years(median 8 years).Since patients were complicated with diabetes,hypertension,cardiac insufficiency and other diseases,transurethral resection of prostate(TURP)was risky Therefore,all patients underwent TUCBDP.Preoperative MRI examination were performed for all patients.CTU examination during urination were performed 1 year after the operations.No intraoperative or postoperative hemorrhage or complications occurred.Patients had unobligated urination 1 year after the operation.Results The preoperative MRI of the 10 patients showed urethral stricture and occlusion of prostatic urethra segment during urination.The CTU performed 1 year after the operation showed that the prostatic urethra was spacious and funnel-shaped.Conclusion MRI combined with CTU can effectively evaluate the changes in prostatic urethral morphology in patients with BPH and provide a reliable scientific basis for clinical TUCBDP.Preoperative MRI during urination can also exclude prostate cancer,and guide the selection of balloon catheter for more accurate measurement of prostate volume,so as to avoid the waste of medical resources.
