摘要
目的分析克氏针与微型钢板内固定术在掌指关节周围骨折患者中的应用效果.方法选取2019年8月—2022年8月本院收治的126例掌指关节周围骨折患者为研究对象,按随机数字表法分为对照组与观察组,每组63例.对照组行克氏针内固定术,观察组行微型钢板内固定术,均观察至术后3个月.对比两组患者的围术期指标、指关节活动度、手部功能恢复情况及并发症发生情况.结果观察组的手术时间为(39.48±2.84)min,长于对照组的(30.25±2.47)min,骨折愈合时间、住院时间分别为(5.23±1.26)周、(5.49±0.90)d,均短于对照组的(7.15±1.84)周、(7.52±1.36)d,组间差异有统计学意义(P﹤0.05).术后,观察组的手指总主动活动度评分为(2.84±0.53)分,高于对照组的(1.72±0.40)分,差异有统计学意义(P﹤0.05).观察组的手部功能恢复优良率为95.24%,高于对照组的84.13%,差异有统计学意义(P﹤0.05).观察组的并发症发生率为3.17%,低于对照组的12.70%,差异有统计学意义(P﹤0.05).结论掌指关节周围骨折患者行微型钢板内固定术,更利于骨折愈合,更好地改善指关节活动度与手部功能,且并发症较少,应用价值较高,值得临床推广使用.
Objective To analyze the effect of Kirschner wire and microplate internal fixation in patients with metacar-pophalangeal fractures.Methods 126 patients with metacarpophalangeal fractures admitted to the hospital from August 2019 to August 2022 were selected as the study objects and were divided into a control group and observation group according to random number table method,with 63 cases in each group.The control group underwent Kirschner wire internal fixation,and the observation group underwent microplate internal fixation,both of which were observed until 3 months after operation.The perioperative indexes,the range of motion of the knuckles,the recovery of hand function and the occurrence of complications were compared between the two groups.Results The operation time of the observation group was(39.48±2.84)min,which was longer than(30.25±2.47)min of the control group,while the fracture healing time and hospitalization time were(5.23±1.26)weeks and(5.49±0.90)d,respectively,which were shorter than(7.15±1.84)weeks and(7.52±1.36)d in the control group,and the differences between the groups were statistically significant(P﹤0.05).After operation,the total active motion score in the observation group was(2.84±0.53)points,which was higher than(1.72±0.40)points in the control group,and the difference was statistically significant(P﹤0.05).The excellent and good rate of hand function recovery in the observation group was 95.24%,higher than 84.13%in the control group,and the difference was statistically significant(P﹤0.05).The incidence of complications in the observation group was 3.17%,lower than 12.70%in the control group,and the difference was statistically significant(P﹤0.05).Conclusion Microplate internal fixation in patients with metacarpophalangeal fractures is more conducive to fracture healing,better improve the range of motion of the knuckle and hand function,has fewer complications and high application value,which is worthy of clinical promotion.
