摘要
目的探究微创钻孔引流术联合高压氧治疗对高血压性脑出血(HICH)患者神经功能相关指标及预后的影响。方法选择2021年6月—2022年7月该院收治的82例HICH患者为研究对象,采用随机数字表法分为对照组和观察组,各41例,对照组采用微创钻孔引流术治疗,观察组采用微创钻孔引流术联合高压氧治疗,两组患者均随访6个月。对比两组神经、意识、血清生化指标及预后情况。结果治疗后,观察组的采用美国国立卫生研究院卒中量表评分为(10.67±1.78)分,低于对照组的(14.69±2.08)分;昏迷恢复量表评分为(19.74±1.02)分,高于对照组的(17.81±2.23)分;观察组的神经元特异性烯醇化酶为(10.75±2.03)μg/L,低于对照组的(15.81±2.11)μg/L;观察组的脑源性神经营养因子、转化生长因子-β_(1)为(18.72±1.73)μg/L、(125.55±12.04)μg/L,高于对照组的(14.29±1.62)μg/L、(100.98±10.56)μg/L;观察组预后良好率为92.68%,高于对照组的73.17%,组间差异有统计学意义(P<0.05)。结论微创钻孔引流术联合高压氧治疗在HICH患者中应用效果更佳,能够有效改善血清生化指标,促进神经功能及意识恢复,改善预后。
Objective To investigate the effect of minimally invasive borehole drainage combined with hyperbaric oxygen therapy on the neurological function-related indexes and prognosis of patients with HICH.Methods 82 patients with HICH admitted to the hospital from June 2021 to July 2022 were selected as the study objects.They were randomly divided into a control group(n=41)and an observation group(n=41).They were treated with minimally invasive drilling and drainage combined with hyperbaric oxygen.Both groups were followed up for 6 months.Results After treatment,the National Institutes of Health Stroke Scale score of the observation group was(10.67±1.78)points,which was lower than(14.69±2.08)points of the control group;the score of coma Recovery Scale was(19.74±1.02)points,higher than(17.81±2.23)points of control group;the neuron-specific enolase(10.75±2.03)μg/L in the observation group was lower than(15.81±2.11)μg/L in the control group;the levels of brain-derived neurotrophic factor and transforming growth factor-β_(1) in the observation group were(18.72±1.73)μg/L and(125.55±12.04)μg/L,which were higher than(14.29±1.62)μg/L and(100.98±10.56)μg/L in the control group(14.29±1.62)μg/L;the good prognosis rate in the observation group was 92.68%,it was higher than 73.17%of the control group,and the differences between groups were statistically significant(P<0.05).Conclusion Minimally invasive borehole drainage combined with hyperbaric oxygen therapy is more effective in patients with HICH,and can effectively improve serum biochemical indexes,promote neurological function and consciousness recovery,and improve prognosis.
