高压氧联合神经肌肉电刺激对脑卒中偏瘫患者神经功能及平衡能力的影响

Effects of Hyperbaric Oxygen Combined with Neuromuscular Electrical Stimulation on Neurological Function and Balance Ability in Patients with Hemiplegia after Stroke

摘要 目的分析高压氧联合神经肌肉电刺激在脑卒中偏瘫患者中的应用价值.方法选取2021年9月—2022年8月我院收治的94例脑卒中偏瘫患者为研究对象,按随机数字表法分为对照组(n=47)和观察组(n=47).两组均接受常规康复训练,在此基础上,对照组采取高压氧,观察组采取高压氧联合神经肌肉电刺激.两组均干预8周.对比两组神经功能、肢体功能、平衡能力、肌电图参数、步行能力.结果干预后,观察组美国国立卫生研究院卒中量表评分、腓肠肌最大积分肌电值(iEMG)、足背屈协同收缩率分别为(9.32±1.09)分、(33.89±2.61)mV·s、(42.73±3.13)%,均低于对照组的(13.14±1.59)分、(36.35±2.88)mV·s、(46.69±3.02)%,Berg平衡量表评分、Fugl-Meyer运动功能评定量表下肢部分评分、胫前肌iEMG分别为(45.56±4.43)分、(26.89±3.13)分、(39.69±3.44)mV·s,均高于对照组的(39.78±3.52)分、(22.11±4.82)分、(36.74±3.21)mV·s,组间差异有统计学意义(P<0.05);干预后,观察组功能性步行量表分级优于对照组,差异有统计学意义(P<0.05).结论高压氧联合神经肌肉电刺激能够促进脑卒中偏瘫患者神经功能恢复,增强肌肉力量,促进肢体功能、平衡能力改善,提升步行能力. Objective To analyze the application value of hyperbaric oxygen combined with neuromuscular electrical stimulation in patients with hemiplegia after stroke.Methods 94 patients with hemiplegia after stroke admitted to the hospital from September 2021 to August 2022 were selected as the research objects,and were divided into a control group(n=47)and an observation group(n=47)according to random number table method.Both groups received routine rehabilitation training.On this basis,the control group received hyperbaric oxygen,and the observation group received hyperbaric oxygen combined with neuromuscular electrical stimulation.Both groups were treated for 8 weeks.The nerve function,limb function,balance ability,electromyography parameters and walking ability were compared between the two groups.Results After the intervention,the National Institute of Health stroke scale score,gastrocnemius integrated electromyography(iEMG)and foot dorsiflexion CO-contraction ratio were(9.32±1.09)points,(33.89±2.61)mV·s and(42.73±3.13)%in the observation group,respectively,which were lower than(13.14±1.59)points,(36.35±2.88)mV·s,(46.69±3.02)%in the control group,Berg balance scale score,Fugl-Meyer assessment-lower extremity score,iEMG of anterior tibial muscle were(45.56±4.43)points,(26.89±3.13)points,(39.69±3.44)mV·s,respectively,which were higher than(39.78±3.52)points,(22.11±4.82)points,(36.74±3.21)mV·s in the control group,and the differences between the groups were statistically significant(P<0.05);after intervention,the classification of functional ambulation category scale in the observation group was better than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Hyperbaric oxygen combined with neuromuscular electrical stimulation can promote the recovery of nerve function,enhance muscle strength,improve limb function and balance ability,and improve walking function in stroke patients with hemiplegia.
