甲巯咪唑联合比索洛尔对甲状腺功能亢进患者甲状腺激素水平及糖脂代谢功能的影响

Effect of Methimazole Combined with Bisoprolol on Thyroid Hormone Levels and Glycolipid Metabolism in Hyperthyroidism Patients

摘要 目的探讨甲巯咪唑联合比索洛尔治疗甲状腺功能亢进(甲亢)的效果.方法选取2020年8月—2022年6月我院收治的96例甲亢患者为研究对象,按随机数字表法分为两组,每组48例.对照组采用甲巯咪唑治疗,观察组在此基础上采用比索洛尔治疗,均持续治疗3个月.对比两组的临床疗效、甲状腺激素水平、糖脂代谢功能、促肾上腺皮质激素(ACTH)和皮质醇(COR)水平、不良反应发生情况.结果观察组的治疗总有效率为95.83%,高于对照组的81.25%,差异有统计学意义(P<0.05).治疗后,观察组的游离甲状腺素、游离三碘甲腺原氨酸水平分别为(4.61±0.79)mmol/L、(4.12±0.84)mmol/L,均低于对照组,促甲状腺激素水平为(3.81±0.59)mmol/L,高于对照组,组间差异有统计学意义(P<0.05).治疗后,观察组的空腹血糖、餐后2 h血糖、糖化血红蛋白、ACTH水平分别为(4.23±0.48)mmol/L、(5.04±0.93)mmol/L、(6.02±0.44)%、(13.12±1.60)pg/mL,均低于对照组,组间差异有统计学意义(P<0.05).治疗后,观察组的低密度脂蛋白胆固醇、总胆固醇、甘油三酯、COR水平分别为(3.69±0.50)mmol/L、(4.95±0.71)mmol/L、(1.18±0.15)mmol/L、(167.31±11.25)nmol/L,均高于对照组,组间差异有统计学意义(P<0.05).两组的不良反应发生率比较,差异无统计学意义(P>0.05).结论甲巯咪唑联合比索洛尔治疗甲亢的效果理想,可有效调节机体甲状腺激素及ACTH、COR水平,改善糖脂代谢,且不良反应少,值得临床推广使用. Objective To investigate the efficacy of methimazole combined with bisoprolol in patients with hyperthyroidism.Methods 96 cases of hyperthyroidism admitted to our hospital from August 2020 to June 2022 were selected as the research objects and were divided into two groups according to random number table method,with 48 cases in each group.The control group was treated with methimazole,and the observation group was treated with bisoprolol on this basis,both of which lasted for 3 months.The clinical efficacy,thyroid hormone levels,glucolipid metabolic function,ACTH and COR levels,and the incidence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 95.83%,which was higher than 81.25%of the control group,the difference was statistically significant(P<0.05).After treatment,the levels of free thyroxine and free triiodothyronine in the observation group were(4.61±0.79)mmol/L and(4.12±0.84)mmol/L,respectively,lower than those in the control group,and the level of thyrotropin in the observation group was(3.81±0.59)mmol/L,higher than that in the control group,the differences between the groups were statistically significant(P<0.05).After treatment,the levels of fasting blood glucose,2 h postprandial blood glucose,glycosylated hemoglobin and ACTH in the observation group were(4.23±0.48)mmol/L,(5.04±0.93)mmol/L,(6.02±0.44)%and(13.12±1.60)pg/mL,respectively,which were lower than those in the control group,the differences between the groups were statistically significant(P<0.05).After treatment,the levels of low density lipoprotein cholesterol,total cholesterol,triglyceride and COR in the observation group were(3.69±0.50)mmol/L,(4.95±0.71)mmol/L,(1.18±0.15)mmol/L and(167.31±11.25)nmol/L,respectively,which were higher than those in the control group,the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Methimazole combined with bisoprolol has an ideal effect in the treatment of hyperthyroidism,which can effectively regulate thyroid hormone,ACTH and COR levels,improve glycolipid metabolism,and reduce adverse reactions.It is worthy of clinical promotion.
