摘要
目的探讨置管溶栓术(CDT)联合支架植入术对髂静脉压迫综合征(Cockett综合征)伴下肢深静脉血栓(DVT)患者血栓清除的影响.方法选取2019年8月—2022年8月临沂市中医医院介入血管科收治的96例Cockett综合征伴DVT患者为研究对象,随机分为观察组(n=48)与对照组(n=48).对照组采用CDT治疗,观察组采用CDT联合支架植入术治疗.对比两组患者的血栓清除率、静脉严重程度、生活质量及术后并发症发生情况.结果观察组的血栓清除率优于对照组,差异有统计学意义(P<0.05).术后,观察组的静脉临床严重程度评分、维拉尔塔评分均低于对照组,组间差异有统计学意义(P<0.05).术后,观察组的慢性下肢静脉功能不全生活质量问卷评分为(86.52±9.44)分,高于对照组的(80.79±8.36)分,差异有统计学意义(P<0.05).两组的术后并发症发生率比较,差异无统计学意义(P>0.05).结论CDT联合支架植入术可提高Cockett综合征伴DVT患者的血栓清除率,减轻静脉严重程度,提升其生活质量,安全性高,值得临床推广使用.
Objective To investigate the effect of catheter-directed thrombolysis(CDT)combined with stent implantation on thrombus clearance in patients with iliac vein compression syndrome(Cockett syndrome)and deep vein thrombosis(DVT)of lower extremities.Methods 96 patients with Cockett syndrome with DVT admitted to the Interventionary Vascular Department of Linyi Traditional Chinese Medicine Hospital from August 2019 to August 2022 were selected as the research objects and were randomly divided into an observation group(n=48)and a control group(n=48).The control group was treated with CDT,and the observation group was treated with CDT combined with stent implantation.The thrombus clearance rate,venous severity,quality of life and postoperative complications were compared between the two groups.Results The thrombus clearance rate in the observation group was better than that in the control group,the difference was statistically significant(P<0.05).After surgery,the venous clinical severity score and Villalta score of the observation group were lower than those of the control group,and the differences between the groups were statistically significant(P<0.05).After surgery,the chronic limb venous iufficiency questionnaire score in the observation group was(86.52±9.44)points,higher than(80.79±8.36)points in the control group,the difference was statistically significant(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion CDT combined with stent implantation can improve the thrombus clearance rate,reduce venous severity and improve the quality of life of Cockett syndrome patients with DVT.It is safe and worthy of clinical application.
