等速肌力训练联合常规康复训练对肩袖损伤患者术后疼痛及关节活动度的影响 被引量:1

Effect of Isokinetic Muscle Strength Training Combined with Conventional Rehabilitation Training on Postoperative Pain and Joint Range of Motion in Patients with Rotator Cuff Injury

摘要 目的探讨等速肌力训练联合常规康复训练在肩袖损伤手术患者中的应用效果.方法选择2020年2月—2022年3月本院收治的肩袖损伤患者84例,按随机数字表法分为两组,每组42例.两组均行关节镜手术进行治疗,术后,对照组进行常规康复训练,观察组在此基础上进行等速肌力训练,两组均持续训练3个月.对比两组的术后疼痛情况、关节活动度及肩关节功能.结果观察组训练后的疼痛视觉模拟评分为(1.72±0.42)分,低于对照组的(2.11±0.46)分,差异有统计学意义(P<0.05).观察组训练后的关节外旋、内旋、前屈及后伸度分别为(79.16±7.21)°、(80.66±5.79)°、(77.18±5.42)°、(76.78±5.22)°,均大于对照组的(72.97±6.33)°、(70.25±5.24)°、(68.63±5.34)°、(65.64±5.71)°,组间差异有统计学意义(P<0.05).观察组训练后的美国加州大学肩关节评分量表中疼痛、功能、前屈角度、前屈肌力及患者满意度评分分别为(8.24±0.87)分、(8.11±1.33)分、(3.44±0.43)分、(3.95±0.43)分、(3.96±0.45)分,均高于对照组的(7.63±1.02)分、(6.95±1.26)分、(2.95±0.46)分、(3.06±0.50)分、(3.22±0.56)分,组间差异有统计学意义(P<0.05).结论等速肌力训练联合常规康复训练在肩袖损伤患者中具有较好的应用效果,利于减轻其术后疼痛,提升关节活动度与肩关节功能. Objective To investigate the effect of isokinetic muscle strength training combined with conventional rehabilitation training in patients undergoing rotator cuff injury surgery.Methods A total of 84 patients with rotator cuff injury admitted to the hospital from February 2020 to March 2022 were divided into two groups according to random number table method,with 42 cases in each group.Both groups underwent arthroscopic surgery.After surgery,the control group received conventional rehabilitation training,and the observation group received isokinetic muscle strength training on this basis.Both groups continued training for 3 months.The postoperative pain,range of motion and shoulder joint function were compared between the two groups.Results The pain visual analogue scale score of the observation group after training was(1.72±0.42)points,lower than(2.11±0.46)points of the control group,the difference was statistically significant(P<0.05).The joint external rotation,internal rotation,anterior flexion and posterior extension of the observation group after training were(79.16±7.21)°,(80.66±5.79)°,(77.18±5.42)°,(76.78±5.22)°,respectively,which were higher than(72.97±6.33)°,(70.25±5.24)°,(68.63±5.34)°,(65.64±5.71)°in the control group,and the differences between the groups were statistically significant(P<0.05).The pain,function,flexor angle,flexor strength and patient satisfaction scores of the university of California at Los Angeles shoulder rating scale in the observation group after training were(8.24±0.87)points,(8.11±1.33)points,(3.44±0.43)points,(3.95±0.43)points and(3.96±0.45)points,respectively,which were higher than(7.63±1.02)points,(6.95±1.26)points,(2.95±0.46)points,(3.06±0.50)points and(3.22±0.56)points in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Isokinetic muscle strength training combined with conventional rehabilitation training has a good application effect in patients with rotator cuff injury,which is conducive to reducing postoperative pain,improving joint motion and shoulder joint function.
