摘要
目的探讨非侵入性脑刺激(NIBS)对轻度认知障碍(MCI)患者整体认知功能的影响.方法检索2022年5月前已发表在PubMed、Cochran Library、中国知网、万方数据和维普数据库关于NIBS治疗MCI患者的文献,最终纳入21篇文献,共373例真刺激者和335例伪刺激者,对两组MCI患者经NIBS治疗后的简易智能精神状态检查量表(MMSE)评分、蒙特利尔认知评估量表中文版(MoCA)评分及不良反应发生率进行Meta分析.结果NIBS能有效改善MCI患者的整体认知功能(SMD=2.00;95%CI=1.34,2.67;P=0.01;I^(2)=84.4%).重复经颅磁刺激(rTMS)能显著改善MCI患者的MMSE评分(SMD=1.92;95%CI=1.30,2.53;P=0.00001;I^(2)=52%),且能显著改善MCI患者的MoCA评分(SMD=2.45;95%CI=1.50,3.41;P=0.00001;I^(2)=94%).NIBS治疗过程中较易出现不良反应(SMD=2.81;95%CI=1.63,4.87;P=0.0002;I^(2)=0%).结论NIBS治疗对MCI患者的整体认知功能有积极影响.
Objective To investigate the effects of non-invasive brain stimulation(NIBS)on global cognitive function in patients with mild cognitive impairment(MCI).Methods The literatures published in PubMed,Cochran Library,CNKI,Wanfang Data and VIP database before May 2022 on the treatment of MCI patients with NIBS were searched.Finally,21 literatures were included,including 373 true stimulators and 335 pseudo-stimulators.Meta-analysis was conducted on the mini-mental state examination(MMSE)score,Montreal cognitive assessment(MoCA)score and the incidence of adverse reactions of MCI patients in the two groups after NIBS treatment.Results NIBS could effectively improve the global cognitive function of MCI patients(SMD=2.00;95%CI=1.34,2.67;P=0.01;I^(2)=84.4%).Repetitive transcranial magnetic stimulation(rTMS)could significantly improve the MMSE score of MCI patients(SMD=1.92;95%CI=1.30,2.53;P=0.00001;I^(2)=52%),and significantly improved the MoCA score of MCI patients(SMD=2.45;95%CI=1.50,3.41;P=0.00001;I^(2)=94%).Adverse reactions were more likely to occur during NIBS treatment(SMD=2.81;95%CI=1.63,4.87;P=0.0002;I^(2)=0%).Conclusion NIBS treatment has a positive effect on the overall cognitive function of MCI patients.
