摘要
目的探讨骨科康复训练在预防人工髋关节置换术(THA)后深静脉血栓形成(DVT)中的应用效果.方法选择2020年4月—2021年6月于该院行THA的患者为研究对象,按随机数字表法将96例患者分为两组,各48例.对照组术后行常规训练,观察组在此基础上行骨科康复训练,两组均持续训练3个月.对比两组DVT发生情况、股动、静脉血流速度、术后恢复情况及髋关节功能.结果两组DVT发生率比较,差异无统计学意义(P>0.05).观察组股静脉血流峰值、平均流速分别为(53.67±3.18)cm/s、(33.31±2.71)cm/s,均快于对照组,股动脉脉血流峰值、平均流速分别为(152.56±11.52)cm/s、(43.46±3.02)cm/s,均快于对照组,组间差异有统计学意义(P<0.05).观察组术后下床活动及住院时间均短于对照组,组间差异有统计学意义(P<0.05).观察组训练后Harris髋关节功能评分中功能、疼痛、关节活动范围、畸形评分分别为(41.46±3.31)分、(39.38±2.87)分、(4.05±0.63)分、(3.05±0.41)分,均高于对照组,组间差异有统计学意义(P<0.05).结论骨科康复训练在预防THA术后DVT方面具有较好的效果,有利于加快股动、静脉血流速度,减少DVT的发生,促进髋关节功能恢复,缩短术后恢复时间.
Objective To investigate the application effect of orthopaedic rehabilitation training in preventing deep vein thrombosis(DVT)after hip arthroplasty(THA).Methods Patients who underwent THA in this hospital from April 2020 to June 2021 were selected as the research subjects,and 96 patients were divided into two groups according to the random number table method,with 48 cases in each group.The control group received routine training after operation,and the observation group received orthopaedic rehabilitation training on this basis.Both groups continued to train for 3 months.The occurrence of DVT,femoral arteriovenous blood flow velocity,postoperative recovery and hip joint function were compared between the two groups.Results There was no significant difference in the incidence of DVT between the two groups(P>0.05).The peak and average velocity of femoral vein blood flow in the observation group were(53.67±3.18)cm/s and(33.31±2.71)cm/s,which were faster than those in the control group,and the peak and average flow velocity of the femoral artery blood flow were(152.56±11.52)cm/s and(43.46±3.02)cm/s,respectively,which were faster than those in the control group,the differences between the groups were statistically significant(P<0.05).The postoperative out-of-bed activity and hospital stay in the observation group were shorter than those in the control group,and the differences between the groups were statistically significant(P<0.05).The scores of functions,pain,range of motion and deformity in Harris hip score of observation group after training were(41.46±3.31)points,(39.38±2.87)points,(4.05±0.63)points,(3.05±0.41)points,respectively,which were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Orthopedic rehabilitation training has a good effect in preventing DVT after THA,which is conducive to accelerating the femoral artery and venous blood flow,reducing the occurrence of DVT,promoting the recovery of hip joint function,and shortening the postoperative recovery time.
