摘要
目的探讨健脾消瘿汤在脾气亏虚型桥本甲状腺炎患者中的应用效果.方法选取该院2020年1—9月收治的52例脾气亏虚型桥本甲状腺炎患者,按随机数字表法分为对照组与观察组,每组26例.对照组采用硒酵母治疗,观察组在对照组基础上加以健脾消瘿汤治疗,连续治疗12周.对比两组临床疗效、治疗前后干扰素-γ(interferon-γ,IFN-γ)、白细胞介素-10(interleukin-10,IL-10)以及甲状腺抗体水平、不良反应.结果两组治疗总有效率进行比较,相比对照组,观察组更高,差异有统计学意义(P<0.05).治疗前,两组IFN-γ、IL-10、甲状腺过氧化物酶抗体(thyroid peroxidase antibody,TPO-Ab)、甲状腺球蛋白抗体(thyroglobulin antibody,TgAb)水平相比,组间差异无统计学意义(P>0.05);治疗后,两组IFN-γ、TPO-Ab、TgAb水平相比,观察组均低于对照组,两组IL-10水平相比,观察组高于对照组,组间差异有统计学意义(P<0.05).观察组不良反应发生率与对照组进行比较,差异无统计学意义(P>0.05).结论健脾消瘿汤能够有效缓解脾气亏虚型桥本甲状腺炎患者的临床症状,调节免疫因子失衡状况,改善甲状腺功能,且不会增加不良反应,安全可靠.
Objective To explore the application effect of Jianpi Xiaoying Decoction in patients with Hashimoto's thyroiditis of spleen qi deficiency type.Methods A total of 52 patients with Hashimoto's thyroiditis of spleen qi deficiency type from January to September 2020 in the hospital were selected and divided into a control group and an observation group according to the random number table method,with 26 cases in each group.The control group was treated with selenium yeast,and the observation group was treated with Jianpi Xiaoying Decoction on the basis of the control group.Both groups were treated for 12 consecutive weeks.The clinical efficacy,interferon-γ(IFN-γ),interleukin-10(IL-10)and thyroid antibody levels before and after treatment,and adverse reactions were compared between the two groups.Results Compared with the control group,the total effective rate of treatment between the two groups was higher in the observation group,and the difference was statistically significant(P<0.05).Before treatment,the levels of IFN-γ,IL-10,thyroid peroxidase antibody(TPO-Ab)and thyroglobulin antibody(TgAb)in the two groups were compared,and the differences between the groups were not statistically significant(P>0.05);after treatment,compared with the levels of IFN-γ,TPO-Ab and TgAb in the two groups,the observation group was lower than the control group,and compared with the IL-10 levels of the two groups,the observation group was higher than the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the observation group and the control group(P>0.05).Conclusion Jianpi Xiaoying Decoction can effectively relieve the clinical symptoms of patients with Hashimoto's thyroiditis of spleen qi deficiency type,regulate the imbalance of immune factors,and improve thyroid function without increasing adverse reactions.It is safe and reliable.
