摘要
目的探讨低频脉冲电刺激时用力吞咽联合咽部冰刺激在脑卒中后吞咽障碍患者康复治疗中的应用效果.方法选取2018年1月—2019年6月该院收治的60例脑卒中后吞咽障碍患者,根据患者入院时间分为观察组(30例)和对照组(30例).患者入院后,给予其常规药物治疗及基础功能训练,基于此,对照组联合咽部冰刺激,观察组在对照组基础上行低频脉冲电刺激时用力吞咽训练,对比两组治疗前后吞咽功能、有效吞咽次数及吞咽速度.结果治疗后,两组的标准吞咽功能评估量表(standardized swallowing assessment,SSA)评分均低于各组治疗前,且观察组低于对照组,组间差异有统计学意义(P<0.05).治疗后,两组的有效吞咽次数均多于各组治疗前,吞咽速度均快于各组治疗前,且观察组的有效吞咽次数多于对照组,吞咽速度快于对照组,组间差异有统计学意义(P<0.05).结论脑卒中后吞咽障碍患者采用低频脉冲电刺激时用力吞咽联合咽部冰刺激,可有效改善吞咽功能,提高有效吞咽次数.
Objective To explore the application effect of forceful swallowing during low-frequency pulse electrical stimulation combined with pharyngeal ice stimulation in the rehabilitation treatment of patients with dysphagia after stroke.Methods 60 patients with dysphagia after stroke admitted to the hospital from January 2018 to June 2019 were selected and divided into an observation group(30 cases)and a control group(30 cases)according to the admission time.After admission,the patients were given routine drug treatment and basic functional training.Based on this,the control group was combined with pharyngeal ice stimulation,and the observation group was given forced swallowing training during low-frequency pulse electrical stimulation on the basis of the control group.The swallowing function,effective swallowing times and swallowing speed before and after treatment were compared between the two groups.Results After treatment,the standardized swallowing assessment(SSA)scores of the two groups were lower than those of each group before treatment,and the scores of observation group werte lower than those of the control group,the differences between the groups were statistically significant(P<0.05).After treatment,the effective swallowing times of the two groups were more than those before treatment,and the swallowing speed were faster than those of each group before treatment,and the effective swallowing times of the observation group were more than that of the control group,and the swallowing speed was faster than the control group,the differences between the groups were statistically significant(P<0.05).Conclusion Patients with dysphagia after stroke use forceful swallowing during low-frequency pulse electrical stimulation combined with pharyngeal ice stimulation,which can effectively improve the patient's swallowing function and increase the number of effective swallows.
