摘要
目的观察早期康复锻炼联合银杏叶提取物在老年患者髋关节置换术后的应用效果.方法选取2019年1月—2020年2月行髋关节置换术的老年患者66例,分为观察组(33例)和对照组(33例).对照组术后常规给予静脉注射帕瑞昔布钠40 mg治疗,观察组在此基础上予以早期康复锻炼联合银杏叶提取物治疗.对比两组治疗前后血清因子水平[载脂蛋白E(ApoE)、白介素-1β(IL-1β)、白介素-6(IL-6)]和功能评分[Harris评分、生活质量量表(SQLS)和蒙特利尔认知量表(MoCA)]情况.结果治疗后,两组IL-1β、ApoE和IL-6水平均有所升高,且观察组均显著低于对照组,差异有统计学意义(P<0.05).治疗后,观察组Harris评分高于对照组,而SQLS评分显著低于对照组,且MoCA评分较治疗前降低程度低于对照组,差异有统计学意义(P<0.05).结论老年患者髋关节置换术后应用早期康复锻炼联合银杏叶提取物,可有效抑制血清炎性水平升高,降低认知功能障碍发生的风险,进而显著提升预后生活质量.
Objective To observe the effect of early rehabilitation exercise combined with Ginkgo biloba extract in elderly patients after hip replacement surgery.Methods 66 elderly patients undergoing hip replacement from January 2019 to February 2020 were selected and divided into an observation group(33 cases)and a control group(33 cases).The control group was routinely given intravenous parecoxib sodium 40mg after operation,and the observation group was treated with early rehabilitation exercise combined with Ginkgo biloba extract on this basis.The serum factor levels[Apolipoprotein E(ApoE),Interleukin-1β(IL-1β),Interleukin-6(IL-6)]and function scores[Harris score,Quality of Life Scale(SQLS)and Montreal Cognitive Scale(MoCA)]of the two groups were compared.Results After treatment,the levels of IL-1β,ApoE and IL-6 in the two groups both increased,and the levels of the observation group were significantly lower than those of the control group,the differences were statistically significant(P<0.05).After treatment,the Harris score of the observation group was higher than that of the control group,the SQLS score was significantly lower than that of the control group,and the decrease of MoCA score before treatment were lower than that of the control group,the differences were statistically significant(P<0.05).Conclusion Early rehabilitation exercise combined with Ginkgo biloba extract after hip replacement in elderly patients can effectively suppress the increase of serum inflammatory levels,reduce the risk of cognitive dysfunction,and significantly improve the prognostic quality of life.
