经会阴超声联合生物反馈评估产后盆底康复的临床价值探讨 被引量:1

Discussion on the Clinical Value of Perineal Ultrasound Combined with Biofeedback in Evaluating Postpartum Pelvic Floor Rehabilitation

摘要 目的 探讨经会阴超声与生物反馈联合检测方法在产后盆底康复评估中的应用价值.方法 选取2018年1月—2019年11月于该院分娩并经产后复检确诊为PFD的62例患者,比较治疗前后盆膈裂孔面积等盆腔超声表现以及盆底肌力变化.结果 治疗后患者静息状态下盆膈裂孔面积明显降低、肛提肌厚度增加,Valsalva状态下膀胱尿道后角缩小,阴道肌电压、Ⅰ类肌纤维持续收缩压以及Ⅱ类肌纤维快速收缩压明显增强,差异有统计学意义(P<0.05).结论 在临床对产后盆底功能障碍性疾病进行康复效果评估时采用经会阴超声联合生物反馈,可更为详尽地获得盆腔形态结构变化及盆底肌力变化情况,临床应用价值较高. Objective To explore the value of perineum ultrasound combined with biofeedback in the evaluation of postpartum pelvic floor rehabilitation.Methods 62 patients who were delivered and diagnosed as PFD after delivery in our hospital from January 2018 to November 2019 were selected.The pelvic ultrasound manifestations such as the area of the pelvic diaphragm hole and the changes of pelvic floor muscle strength before and after treatment were compared.Results After treatment,the area of phrenic hole decreased significantly,the thickness of levator ani muscle increased,the posterior angle of bladder and urethra decreased.The voltage of vaginal muscle,the continuous systolic pressure of type I muscle fiber and the rapid systolic pressure of type II muscle fiber increased significantly,the differences were statistically significant(P<0.05).Conclusion In the clinical evaluation of the rehabilitation effect of postpartum pelvic floor dysfunction,transperineal ultrasound combined with biofeedback can be used to get more detailed changes of pelvic morphology and structure and the changes of pelvic floor muscle strength,which is of high clinical value.
